浅析肾阳虚型腰背痛腰椎CT影像表现

摘　　要：目的肾阳虚型腰背痛为腰背痛的常见类型,为了提高治疗效果,做到有的放矢,希望对此型腰背痛患者的腰椎及其周围结构的病变用影像学的手段加以了解。方法对84例肾阳虚型腰背痛患者腰椎CT结果进行分析。结果 CT结果显示:出现椎体骨质增生的有56例(占67%),椎小关节退变的有28例(占33%),椎体滑脱的有13例(占15%),椎间盘膨出的有63例(占75%),椎间盘突出的有21例(占25%),椎间盘"真空征"的有7例(占8%),硬膜囊明显受压的有42例(占50%),双侧神经根受压的有42例(占50%)。肾阳虚型腰背痛的患者腰椎可出现椎间盘异常、腰椎椎体及椎小关节有退行性改变、腰椎滑脱表现,其中椎间盘膨出所占比例最大。结论肾阳虚型腰背痛的患者可出现腰椎、椎间盘的退行性改变,这些是导致患者腰背痛的原因之一,对于临床上肾阳虚型腰背痛的患者的治疗提供一定的依据。Objective Lumbar and back pain of kidney yang deficiency type is a common type of lumbar and back pain. In order to improve the therapeutic effect, understanding CT image manifestation of the lumbar vertebrae and its surrounding structure in patients with low back pain is important. Methods CT findings of lumbar vertebrae in 84 patients with low back pain due to deficiency of kidney yang were analyzed. Results There were 56 cases （67%） with bone hyperplasia in vertebral body, 28 cases （33%） with degeneration of facet joints, 13 cases （15%） with spondy- Iolisthesis, 63 cases （75%） with herniation of intervertebral, 21 cases with disc herniation （25%）, and 7 cases, ＂vacuum sign＂ of intervertebral disc （8%）, 42 cases （50%） with obvious compression of dural sac and 42 cases （50%） with bilat- eral nerve root compression. The lumbar vertebrae and facet joints have degenerative changes and lumbar spondylolis- thesis is the most common phenomenon in the patients with kidney yang deficiency and low back pain. Conclusion Degenerative changes of lumbar vertebrae and intervertebral disc may occur in patients with low back pain due to defi- ciency of kidney yang, which is one of the causes of low back pain in patients with kidney yang deficiency. Theresult- sprovide some basis for the treatment of patients with low back pain due to deficiency of kidney yang.
