摘 要:目的探讨T2DM患者血清邻接锌指基因1(JAZF1)、内脂素(Visfatin)水平与大血管病变的相关性。方法选取2014年9月至2016年1月于遵义医学院附属医院住院的T2DM合并大血管病变患者(T2DM+M组)32例及单纯T2DM患者(T2DM组)36例;同期选取我院健康体检人群(NC组)40名。ELISA测定各组血清JAZF1及Visfatin水平,分析各组临床资料、JAZF1及Visfatin的差异及相关性。结果 T2DM+M组血清JAZF1水平较NC组降低(P<0.05);T2DM+M组血清Visfatin水平较T2DM组及NC组升高(P<0.05)。Pearson相关性分析显示,血清JAZF1与Visfatin、TG、WHR呈正相关(P<0.05或P<0.01);血清Visfatin与JAZF1、FPG、2hPG、TG、WHR呈正相关(P<0.05或P<0.01)。多元线性回归分析结果显示,Visfatin是JAZF1的独立影响因素;JAZF1、TG、2hPG是Visfatin的影响因素。二元Logistic回归分析结果显示,JAZF1、Visfatin、FPG、WHR是T2DM大血管病变的影响因素。结论 JAZF1、Visfatin与糖脂代谢及T2DM大血管病变密切相关。Objective To investigate the correlation between serum JAZF1, Visfatin and macroangiopathy in patients with type 2 diabetes mellitus(T2DM). Methods A total of 68 patients with T2DM who were hospitalized in Affiliated Hospital of Zunyi Medical University were selected in this study from Sep 2014 to Jan 2016.All the patients were divided into two groups:T2DM with macroangiopathy(T2DM+M group,n=32)and T2DM without macroangiopathy(T2DM group,n=36).Meanwhile,healthy subjects were selected as the normal control group(NC group,n=40).The serum JAZF1 and Visfatin were determined by enzyme linked immunosorbent assay(ELISA).The differences and correlation of clinical data,JAZF1 and Visfatin were analyzed among the groups. Results The serum level of JAZF1 were significantly lower in T2DM+M group than in NC group(P0.05).The serum level of Visfatin were significantly higher in T2DM+M group than in T2DM group and NC group(P0.05).Pearson correlation analysis demonstrated that the serum JAZF1 was positively correlated with Visfatin,TG and WHR(P0.05 or P0.01);the serum Visfatin was positively correlated with JAZF1,FPG,2 hPG,TG and WHR(P0.05 or P0.01).Multivariate linear regression indicated that Visfatin was an independent influencing factor for JAZF1 and JAZF1,TG and 2 hPG were influencing factors for Visfatin.Logistic regression analysis showed that JAZF1,Visfatin,FPG and WHR were the most significant factors in the development of type 2 diabetic macroangiopathy. Conclusion JAZF1 and Visfatin were closely related to glucose and lipid metabolism,which may play an important role in the pathogenesis of type 2 diabetic macroangiopathy.
