摘 要:目的开展并评价尿C-P/肌酐比(UCPCR)的检测方法及影响因素。方法测定尿C-P(UCP)检测试剂盒的批内和批间CV;评估不同温度保存方式、保存时间和干扰物存在情况下,UCP检测结果的变化规律。测定北京地区20名正常人、20例T1DM患者及20例T2DM患者的空腹UCPCR,比较其差异。结果 UCP批内CV为0.78%,批间CV为4.02%;T1DM患者空腹UCPCR低于T2DM患者和正常人[0.01(0.00,0.11)vs 0.28(0.20,0.48)vs 0.86(0.54,1.44)nmol/mmol,P<0.001];保存于2~8℃时,UCP第3天时衰减10%。保存于-80℃时,第5个月时衰减8%;尿液中蛋白浓度≤6.8g/L时,UCP干扰率为8.3%,尿液中红细胞≤57500个/μl时,UCP干扰率为7.5%。结论 UCPCR能反映胰岛β细胞功能。UCP在-80℃稳定,受一般蛋白尿及肉眼血尿干扰小。Objective To evaluate the practicability and influencing factors for urinary C-peptide/creatinine ratio(UCPCR)measurement. Methods Within-run and between-run analysis were conducted and coefficient of variation(CV)were calculated.The urinary C peptides(UCP)were tested at different conditions such as preservation temperatures,preservation durations and interfering substances.UCPCR was measured in 20 healthy subjects,20 patients with type 1 diabetes(T1DM)and 20 patients with type 2 diabetes(T2DM). Results The CVs for within-run and between-run were 0.78% and 4.02%.The UCPCRs[0.01(0.00,0.11)nmol/mmol]were lower in patients with T1DM than in patients with T2DM[0.28(0.20,0.48)nmol/mmol]and healthy subjects[0.86(0.54,1.44)nmol/mmol](P0.001).At 2~8℃,for 72 hours,the reduction of UCP was 10%.At-80 ℃ was 8% within five months.The interfering rates for the urine with protein(≤6.8 g/L)and blood red cells(≤57500 piece/μl)were 8.3%and 7.5%,respectively. Conclusion UCPCR is a marker ofβcell function.UCP is stable even though the urine is preserved at-80℃ and interfered by protein and blood.
