摘 要:目的探讨血清中卵泡生成素(FSH)和睾酮(T)水平与患者精子质量和男性不育的关系。方法回顾性分析2015年9月-2017年10月在本院行精子质量检查的100例已婚男性的临床资料,采用电化学发光法检测血清中FSH和T水平,分析FSH和T与精子质量的关系,采用Logistic分析探讨FSH和T对男性不育的预测价值。结果以FSH、T水平中位数为界分为高水平组和低水平组,结果发现FSH高水平组、T低水平组精液体积、精子密度、精子活动率、正常精子形态均低于FSH低水平组、T高水平组(P <0. 05)。Pearson相关性分析显示,FSH水平与精子密度和精子活动率呈负相关(r值分别为-0. 584和-0. 428,P <0. 05),T水平与精子密度和精子活动率呈正相关(r值分别为0. 427和0. 589,P <0. 05)。Logisitic回归分析结果显示,精子密度(P=0. 013)、精子活动率(P=0. 012)、T(P=0. 030)、FSH(P=0. 020)是男性不育的独立风险因素。结论血清中FSH和T水平与精子质量密切相关,是导致男性不育的独立风险因素,值得临床重视。Objective To investigate the relationship between serum levels of follicle-stimulating hormone( FSH) and testosterone( T) and sperm quality and male infertility. Methods The sperm quality inspection of clinical data in 100 married male from September 2015 to October 2017 in our hospital were retrospectively analyzed. Serum FSH,T levels were detected with electrochemiluminescence immunoassay,and the relationship between FSH,T and sperm quality was analyzed,and logistic analysis was used to evaluate the value of FSH and T in prediction of male infertility. Results The median FSH and T levels were used to divide the subjects into high-level group and low-level group. The results showed that the semen volume,sperm density,sperm motility and normal sperm morphology of high FSH level group and low T level group were lower than that of the low FSH level group and high T level group( P 0. 05). Pearson correlation analysis showed that there was a negative correlation between FSH,sperm density and sperm activity rate( r =-0. 584,r =-0. 428,P 0. 05),while T level was positively correlated with sperm density and sperm activity rate( r = 0. 427,r = 0. 589,P 0. 05). Logistic regression analysis suggested that sperm density( P = 0. 013) and sperm motility( P = 0. 012),T( P = 0. 030),FSH( P = 0. 020) were independent risk factors for male infertility. Conclusion There was a negative correlation between FSH,T and sperm quality,which is an independent risk factor for male infertility. It is worth clinical attention.
