摘　　要：目的初步探讨腹腔镜辅助下改良Soave术式治疗大龄儿童先天性巨结肠症的治疗效果。方法以湖南省儿童医院2012年2月至2018年1月接受腹腔镜辅助下改良Soave术式治疗的10例大龄(年龄7岁10个月至12岁6个月)先天性巨结肠症患儿为研究对象,并结合患儿的临床资料及治疗效果进行评估。结果 10例均顺利完成手术。手术时间120～180 min,平均150 min。均在术后1个月开始行扩肛治疗,术后出现污粪2例,半年后症状消失。结论从本次纳入10例患儿的治疗效果来看,腹腔镜辅助下改良Soave术式治疗大龄儿童先天性巨结肠症具有疗效好、创伤小、恢复快、外观美观的特点。Objective To explore the efficacies of modified Soave with the aid of laparoscopy in thetreatment of Hirschsprung＇s disease （HD） in older children. Methods Retrospective analysis was performedfor 10 HD cases undergoing modified Soave with the aid of laparoscopy. The age range was 94 - 150 months.Results All operations were successfully completed. The median operative duration was 150（120 - 180） min.Two cases of postoperative fecal soiling underwent anusexpanding procedures at 1 month and the symptom disappeared at 6 months. Conclusion For HD in older children,modified Soave with the aid of laparoscopy hasthe advantages of excellent efficacies,minimal operative trauma, rapid recovery and excellent cosmetic outcomes.
