摘  要:目的探讨大叶性肺炎患儿诱导痰液中T淋巴细胞亚群、细胞因子的变化及其临床意义。方法选取我院确诊的大叶性肺炎患儿148例(肺炎组),收治时间2015年8月-2017年8月,另选取健康儿童80名作为对照组,检测两组诱导痰液中的T淋巴细胞亚群、细胞因子水平,并根据临床肺部感染评分(CPIS)将肺炎组分为重症组和轻症组进行亚组分析。结果肺炎组诱导痰液中CD3^+、CD4^+、CD4^+/CD8^+的水平均显著高于对照组(P<0.05);肺炎组诱导痰液中的IL-6、IL-8、TNF-α的水平均显著高于对照组(P<0.05);轻症组诱导痰液中CD3^+、CD4^+、CD4^+/CD8^+的水平均显著低于重症组(P<0.05);轻症组诱导痰液中的IL-6、IL-8、TNF-α的水平均显著低于重症组(P<0.05)。结论大叶性肺炎患儿诱导痰液中T淋巴细胞亚群、细胞因子水平升高显著,提示患儿免疫功能紊乱,并且与患儿病情变化相关。Objective To investigate the changes of cytokines in bronchoalveolar lavage fluid (BALF) and the changes ofT lymphocyte subsets in induced sputum in children with lobar pneumonia and their clinical significance. Methods A total of 148cases of children diagnosed with lobar pneumonia and treated in our hospital between August 2015 and August 2017 were selectedas the pneumonia group, while another 80 healthy children served as the control group. Cytokines in BALF and T lymphocytesubsets in induced sputum were detected. Patients in the pneumonia group were again divided into severe pneumonia group andmild pneumonia group according to the clinical pulmonary infection score ( CPIS) . Results The levels of CD3+, CD4+andCD4+/ CD8+in sputum were significantly higher in the pneumonia group than in the control group (P〈0. 05), so were the levelsof IL-6, IL-8 and TNF-琢. The levels of CD3+, CD4+and CD4+/ CD8+in sputum were significantly lower in the induced pneumonia group than in the control group (P〈0. 05). The levels of IL-6, IL-8 and TNF-琢 in sputum were significantly lower in themild group than those in the severe group (P〈0. 05). Conclusion Levels of T lymphocyte subsets and cytokines in induced sputum increase significantly in children with lobar pneumonia , suggesting that children may suffer from immune disorders. Theselevels may be associated with changes in the conditions of children.
