丹桂三香汤联合氨氯地平治疗冠心病的临床研究

摘　　要：目的观察丹桂三香汤联合氨氯地平治疗冠心病的临床疗效及对心肌损伤指标、自主神经功能的影响。方法选取2014年5月—2017年5月我院收治的120例冠心病病人,按照随机数表法分为试验组和对照组,每组60例。对照组采用氨氯地平治疗,试验组采用丹桂三香汤联合氨氯地平治疗。比较两组心电图疗效、临床疗效;观察两组治疗前后心肌损伤指标及心率变异性(HRV)指标变化情况。结果治疗后,试验组临床疗效总有效率为93.33%,高于对照组的80.00%,差异有统计学意义(P <0.05);试验组心电图疗效总有效率为78.33%,高于对照组的58.33%,差异有统计学意义(P <0.05)。两组治疗后心肌损伤指标均上升,但对照组上升程度明显大于试验组(P <0.05)。两组治疗后HRV各指标均较治疗前明显上升,QT离散度(QTd)则显著下降,且试验组HRV、QTd改善程度优于对照组(P <0.05)。结论丹桂三香汤联合氨氯地平治疗冠心病,可明显提高临床疗效和心电图疗效,对心肌具有更好的保护作用,且对病人自主神经功能影响较小。Objective To study effects of Dangui Sanxiang Decoction combined with Amlodipine on cardiac protection in patients withcoronary heart disease and its effects on autonomic nerve function.Methods One hundred and twenty patients with coronary heartdisease from May 2014 to May 2017 were divided randomly into control group and experiment group.The control group was given Am -lodipine,the experiment group was given Dangui Sanxiang Decoction combined with Amlodipine.Clinical effect, electrocardiogram,changes in myocardial injury,and changes in heart rate variability （HRV） were compared between two groups.Results After treat -ment,the total effective rate （93.33%） in the experimental group was higher than that in the control group （80.00%）,with statisticallysignificant difference（P 〈0.05）.The total effective rate of electrocardiogram was 78.33% in the experimental group and 58.33% in thecontrol group, with statistically significant difference between groups （ P 〈 0. 05）. The levels of troponin （ cTnI ）, creatine kinaseisoenzyme （CK- MB）,brain natriuretic peptide （BNP）,and serum superoxide dismutase （SOD） were significantly increased in bothgroups,that in the control group was significantly higher than that of the experimental group （ P 〈0.05）.RMSSD,SDNN index,SDNN,and PNN50 were significantly increased in two groups,QTd significantly decreased,and the improvement in the experimental groupwas significantly higher than that in the control group （P 〈0.05）.Conclusion Dangui Sanxiang Decoction combined with amlodipinein the treatment of coronary heart disease was significantly better than amlodipine alone,showing better protective effect on myocardi-um,and less influence on the autonomic nerve function.
