子宫内膜息肉电切术后放置曼月乐环与口服地屈孕酮预防复发的对比研究

摘　　要：目的对比研究子宫内膜息肉电切术后放置曼月乐环及口服地屈孕酮预防复发的效果。方法选取2016年1月-2017年6月之间在我院因内膜息肉行宫腔镜下息肉电切术的患者共106例,曼月乐组:宫内放置左炔诺孕酮宫内缓释系统(曼月乐环)为35例;地屈孕酮组:口服地屈孕酮后半周期治疗3月,共34例,对照组:37例;观察上述患者术后3、6、12月B超、月经量、经期情况,对比两组的治疗效果及追踪复发率的情况。结果随访期间曼月乐组、地屈孕酮组的复发率、月经失血量评分(PBAC)及子宫内膜厚度低于对照组,P<0.05,术后曼月乐组和地屈孕酮组不同时间的上述指标无明显差异,P>0.05。结论子宫内膜息肉电切术后放置曼月乐环或者口服地屈孕酮均可有效降低子宫内膜息肉的复发率,值得推广。Obejcetive To compare the effect of LNG-IUD with that of oral Dydrogesterone in preventing recurrence after endometrial polyp electrocision.Methods 102 patients who underwent the endometrium polyp electroci-sion between January 2016 and June 2017 were selected in our hospital. 35 cases were allocated into the MLR group（LNG-IUD group）, and 34 cases who took the Dydrogesterone in the last ten days of the menstrual cycle for 3 monthswere allocated into the Dydrogesterone group. Control group： 37 cases.Ultrasonography, menstrual volume （PBAC method） and menstrual bleeding time were used to evaluate the effect of the two groups at the 3rd , 6th and 12th monthafter the surgery. Results The recurrence rate, PBAC and the thickness of endometrium were lower than the controlgroup. during the follow-up period, while the menstrual bleeding time was higher than the control group（P 〈0.05）.There was no significant difference between the LNG-IUD group and the Dydrogesterone treatment group at eachfollow-up time（ P〉0.05）.Conclusion The placement of LNG-IUD or oral progesterone （Dydrogesterone） after en-dometrial polyp electrotomy can effectively reduce the recurrence rate of endometrial polyps, and are worthy ofpromotion in clinic.
