摘　　要：用药交代工作是临床治疗的重要一环,但受多种因素的影响,目前,用药交代存在不提供或提供不到位等问题。用药交代是药师的职责,需采取行之有效的措施实现精准用药。实行分级用药交代的模式,不仅能提升药学服务的质量,还能提高患者的用药依从性,从而提高药物的治疗效果。分级用药交代模式是一种有效、可操作性强的技术策略,可基本实现精准用药,值得在广大医院推广。Medication account is an important part of clinical treatment,however,under the influence of many factors,there are some problems,such as,it＇ s not provided or it＇ s not available in the right place for medication account. M edication account is the responsibility of pharmacists,so we need to take effective measures to achieve accurate medication. The implementation of the mode of graded drug delivery can not only improve the quality of pharmaceutical services,but also enhance the patient＇s compliance to drugs,so as to improve the therapeutic effect of drugs.The model of graded medication account is an effective and workable technical strategy and makes medication basically accurate which is worth promoting in hospitals.
