替考拉宁治疗2型糖尿病合并革兰阳性球菌肺炎疗效分析

摘　　要：目的评价替考拉宁治疗2型糖尿病合并革兰阳性球菌社区获得性肺炎患者的临床疗效及安全性。方法采用回顾性研究的方法,分析2014年-2017年于医院住院的2型糖尿病合并社区获得性肺炎的病例,确诊及高度怀疑革兰阳性球菌肺炎并使用替考拉宁治疗的社区获得性肺炎患者共72例,根据痰培养结果分为痰检阳性组和痰检阴性组,观察其临床疗效、细菌清除率及不良反应发生率等。结果 72例患者替考拉宁的使用疗程为8～14d,临床疗效总有效率88.89%,痰检阳性组与痰检阴性组总有效率无统计学差异,其中痰检阳性组的细菌转阴率为87.75%,72例患者中,患者血糖及感染均得到有效控制,72例患者中不良反应率6.94%。结论替考拉宁能有效治疗2型糖尿病合并革兰阳性球菌社区获得性肺炎,不良反应较少。OBJECTIVE To evaluate the clinical efficacy and safety of teicoplanin in treatment of type 2 diabetes mellitus patients complicated with community-acquired pneumonia（CAP）caused by gram-positive cocci.METHODS By means of retrospective study,the type 2 diabetes mellitus patients complicated with CAP who were hospitalized from 2014 to 2017 were observed,totally 72 patients who were diagnosed and highly suspected with gram-positive cocci pneumonia and were treated with teicoplanin were recruited as the study objects and divided into the positive sputum culture group and the negative sputum culture group according to the result of sputum culture.The clinical curative effect,bacterial clearance rate and incidence of adverse reactions were observed.RESULTS The treatment course of teicoplanin of the 72 patients varied from 8 to 14 days,the total effective rate of clinical treatment was 88.89%.There was no significant difference in the total effective rate between the positive sputum culture group and the negative sputum culture group,the negative rate of bacteria of the positive sputum culture group was 87.75%.Among the 72 patients,The blood glucose and the infection in 69.40% of the patients were effectively under control.The incidence of adverse reactions was 6.94% among the 72 patients.CONCLUSIONTeicoplanin can achieve remarkable effect on treatment of type 2 diabetes mellitus patients complicated with CAP caused by gram-positive cocci,and the incidence of adverse reactions is low.
