摘　　要：我国已进入深度老龄化社会,探索具有中国特色的医养结合服务体系具有重大的现实意义。医疗模式以健康管理、中医调理、康复护理相结合,突出中医治未病的传统优势,强调主动干预、持续跟踪、培训老人的自我健康管理和养生保健技能,共服务老人121例,取得满意效果,以资借鉴。As China has entered the phase of deep ageing,it is of great practical importance to explore a Chinese style medicare and nursing service system with the combination of health management,traditional Chinese medicine（TCM） and rehabilitation nursing.Taking advantage of the prevention treatment for disease in TCM,and with the active intervention,continuous monitoring,and the elderly＇s self heath management and health care skills training,we have offered workable and satisfactory solutions to 121 senior patients.The experience will be discussed in the following.
