摘　　要：目的对比应用切割器和等离子治疗儿童睡眠呼吸暂停综合征的安全性及其临床疗效。方法选取2017年3月～2017年12月本科收治的由于腺样体肥大引起的睡眠呼吸暂停低通气综合征患儿300例,随机分为两组,150例行鼻内镜下行腺样体电动切割器切除术(切割器组),另外150例行鼻内镜下低温等离子切除术(等离子组)。对比两组手术时间、术中出血量、术后疼痛程度、术后出血(原发和继发)情况、手术疗效等指标。结果等离子组在手术时间、出血量(x±s)、术后疼痛程度、术后出血率等方面明显低于切割器组(P<0.05),但两组在术后6个月的疗效比较差异无统计学意义。结论相比较切割器切除腺样体而言,等离子腺样体切除术缩短手术时间、减少术中出血少、减轻术后疼痛、降低术后出血率,值得临床广泛应用。Objective Comparative the safety and clinical efficacy between the application of electric cutter and low temperature plasma in the treatment of children with OSAHS（obstructive sleep apnea-hypopnea syndrome）. Methods A total of 300 children with OSAHS caused by adenoid hypertrophy from March 2017 to December 2017 in our department, were randomly divided into two groups, 150 patients with electric cutter of endoscopic adenoidectomy （cutter group）, other 150 patients with low-temperature plasma of endoscopic adenoidectomy （plasma group）. Operation time, blood loss, postoperative pain, postoperative bleeding （primary and secondary）, and operative efficacy were compared between the two groups. Results Plasma group was significantly lower than that of the cutter group in operation time, blood loss （x ± s）, postoperative pain, postoperative bleeding rate （P〈0.05）, but there was no significant difference between the two groups in postoperative efficacy of 6 months. Conclusion Compared with the adenoidectomy with cutter, adenoidectomy with plasma can shorten the operation time, reduce the intraoperative blood loss, reduce postoperative pain, reduce the rate of postoperative bleeding, it is worth to be widely used in clinical.
