医药价格综合改革对三甲公立医院运营的影响——以湖北省某三甲医院为例

摘　　要：[目的]研究医药价格综合改革对医院医疗收入总额和医疗收入结构的影响,为医药价格综合改革背景下三甲公立医院运营提供参考。[方法]采用因素分析法和结构变动度分析法研究湖北省某三甲医院取消药品加成前后一年的医疗收入总额及医疗收入的结构变化。[结果]药品零加成后湖北省某三甲医院的医疗收入总额同比增长26. 27%;患者数量的增长对门诊及住院收入影响的比例分别是89. 80%、73. 38%,其对收入的贡献率大于次均费用的增长;在门诊和住院收入中,药品收入分别下降4. 66%和6. 66%,药占比下降明显,但未达改革目标。[结论]医药价格改革后,医院的整体运营平稳,患者数量增长是门诊和住院收入增长的主要原因,药占比下降,体现医务人员劳动价值的技术、治疗占比提高,医疗收入结构进一步优化。Objective To study the influence of total amount and hospital medicine income structure before and after the comprehensive medicine price reform so as to provide reference for the operation of tertiary public hospital under the background of comprehensive medicine price reform. Methods It studied total amount and hospital medicine income structure change in the year before and after drug price addition in a tertiary public hospital in Hubei Province by factor analysis and structural change analysis method. Results The total medicin income of a tertiary hospital in Hubei Province increased by 26.27% after zero addition of drugs. The proportion of the number increasing of patients which influenced the income of out-patient and hospitalization was 89.80% and 73.38% respectively,its rate of contribution was bigger than the increase of secondary average expense. In the income of out-patient and hospitalization,drug income decreased 4.66% and 6.66% respectively,it decreased obviously but didn t reached reform goal. Conclusions After medicine reform,overall operation of hospital is smooth and steady. The number increase of patients is the main factor of the income of out-patient and hospitalization. The proportion of drug decreases,the technique which reflect medical staffs value of labour power and the proportion of treatment increase. Medicine income structure further optimizes.
