老年2型糖尿病肾病患者尿液足细胞血清胱抑素C血清同型半胱氨酸水平与肾功能的相关性

摘　　要：目的探讨老年2型糖尿病肾病患者尿液足细胞、血清胱抑素C (Cys-c)、血清同型半胱氨酸(Hcy)水平与肾功能的相关性。方法选取2015年6月至2017年6月首都医科大学附属地坛医院收治的98例老年2型糖尿病肾病患者,纳入观察组。选取同期于我院行健康体检的98例老年健康志愿者为对照组。比较两组患者尿液足细胞、血清Cys C、Hcy水平与肾功能检测等相关生化指标的差异,采用Pearson相关性分析尿液足细胞、血清Cys C、Hcy水平与肾功能指标的相关性。结果观察组患者尿液足细胞、血清Cys C、Hcy水平均高于对照组,差异有统计学意义(P <0. 05),肾功能指标:血肌酐(Cr)、血清尿素(BUN)、血清β2微球蛋白(β2-MG)、尿酸(UA)均高于对照组患者,差异有统计学意义(P <0. 05)。Pearson相关性分析显示老年糖尿病患者尿液足细胞、血清Cys C、Hcy水平与患者肾功能指标(Cr、BUN、β2-MG、UA)均呈正相关(r> 0,P <0. 05)。结论老年2型糖尿病患者尿液足细胞、血清Cys C、Hcy水平与肾功能呈正相关。Objective To investigate the relationship between serum cystatin C （Cys-c）, serum homocysteine （Hcy） levels and renal function in elderly patients with diabetes mellitus. Methods The research enrolled 98 elderly patients with diabetic nephropathy admitted to the Ditan Hospital of Capital Medical University from June 2015 to June 2017. The control group selected 98 elderly healthy volunteers admitted to our hospital for physical examination. The differences in urinary podocytes, serum Cys C, Hcy levels and renal function tests were compared between the two groups. Pearson correlation analysis was used to assess the index correlation. Results The urinary podocytes, serum Cys C and Hcy levels in study group were higher than those in control group （ P 〈 0.05）. Renal function indicators, such as urine creatinine （Cr）, serum urea （BUN）, serum β2 microglobulin （β2-MG） Uric acid （UA） were higher than those of control group （ P 〈 0.05 ）. The levels of urinary podocytes, serum Cys C and Hcy in elderly diabetic patients were positively correlated with renal function indexes （Cr, BUN, β2-MG, UA） （ r 〉 0, P 〈0.00）. Conclusion The levels of urinary podocytes, serum Cys C and Hcy in elderly diabetic patients are positively correlated with renal function.
