基于医院视角的曲美他嗪缓释片经济性研究

摘　　要：目的:探讨曲美他嗪缓释片替换常释片后,对患者和医院相关成本可能造成的影响。方法:基于医院角度,对于稳定性心绞痛患者,从药物成本、人力成本两个维度,分析不同曲美他嗪剂型之间的替换对每人次每天的住院成本和医院总成本可能造成的影响。结果:在个人层面,曲美他嗪缓释片替代常释片后,每人次每天的住院成本可节约0.71元。其中曲美他嗪成本增加1.05元、硝酸甘油成本降低0.04元、护理的时间成本降低1.72元。在医院层面,当缓释片替代率由25%提升至100%时,每年全国医院在稳定性心绞痛上预计节约65.61～262.43万元。结论:在按病种付费背景下,使用曲美他嗪缓释片替代常释片有助于医院节约和稳定性心绞痛相关的成本。Objective： To investigate the differences of patients and hospital-related cost before and after the substitution of Trimetazidine tablets with Trimetazidine Modified release（MR） tablets. Methods： From the hospital＇s perspective, the impact of switching from trimetazidine tablets to trimetazidine MR tablets in the stable angina pectoris patients were analyzed from the density of pharmaceutical cost and labor cost. Results： In the individual level, the total cost per patient’s admission would be reduced by 0.71 yuan^metazidine＇s cost increased by 1.05 yuan, nitroglycerin cost reduced by 0.04 yuan and time cost of nursing care decreased by 1.72 yuan. In the hospital level, if the substitution rate increased from 25% to 100%, the hospital might save 0.6561-2.6243 million yuan in average. Conclusion： Using trimetazidine MR tablets might be helpful in saving hospital-related cost under DGRs payments.
