热敏灸感条目德尔菲法调查分析

摘　　要：目的筛查基于热敏灸理论及其技术的热敏灸感条目,为进一步建立热敏灸得气灸感量表提供科学依据。方法通过文献检索和访谈法,对灸感词汇进行收集,建立备选条目池;通过德尔菲法对来自6个省的25名专家进行2轮问卷调查,根据积极程度、权威程度、集中程度、协调程度进行统计、评价和筛选热敏灸感条目。结果通过文献检索纳入条目39项,访谈法纳入35项,删除重复的6项,共40项热敏灸感条目进入调查。第一轮发放问卷25份,回收20份,回收率80%;第二轮发放问卷20份,回收18份,回收率90%。删除重要性得分在3分及以下者,第1轮14项,第2轮1项,最终筛查出热敏灸感条目为透热、表面不热(或微热)深部热、传热、扩热、局部不热(或微热)远部热、酸感、胀感、痛感(非施灸局部灼痛感)、风吹感、流水感等25项。其变异系数均<0. 25,专家权威程度平均为(0. 772±0. 067)分。结论本次调查分析专家积极度高、权威度高,所获透热、传热、扩热、肢端热、身烘热、喜热等25项灸感可作为热敏灸临床诊疗评估的热敏灸感条目。Objective To screen Heat-sensitive Moxibustion（HSM） sensation items based on Heat-sensitive Moxibustion theory and its technology to provide a scientific basis for further establishment of HSM sensation scale.Methods The items of sensation descriptors were collected to establish the pool of alternative items by documentary method and interviewing. By two rounds of questionnaire survey of 25 experts from 6 provinces with Delphi method,the data were statistically analyzed and evaluated in the aspects of positive level,concentration,coordination,authoritativeness. Results A total of 40 HSM items were included in the survey. The items of HSM sensation were summarized from the first 39 screened items and 35 items from interviewing,with omissions of 6 same items. In the first round,25 questionnaires were distributed,and 20 were withdrawn. The recovery rate was 80%. In the second round,20 questionnaires were distributed and 18 were withdrawn,with a recovery rate of 90%. Questionnaires with importance score of 3 points or less were deleted,which in the first round were 14 items and the second round was 1 item. Finally the heat-sensitive moxibustion items as heat penetrating,surface heat（or micro-heat） deep heat,heat transferring,heat spreading,not local heat（or micro-heat） but distal heat,sourness,distension sensation,pain（not moxibustion,local burning pain）,wind sensation,running water sensation,etc. All together 25 items were screened. The coefficient of variation was 0. 25,and the average degree of expert authority was（0. 772 ± 0. 067）points. Conclusion A total of 25 moxibustion sensations,such as heat penetrating,heat spreading,heat transferring,acra heat,body heat freshening,heat preference,can be used as HSM sensation items in HSM clinical diagnosis and treatment.
