尿沉渣、尿干化学及显微镜3种方法检测尿液白细胞的对比分析

摘　　要：目的对比分析Sysmex UF1000i全自动尿沉渣分析仪、Arkray AX-4030尿干化学分析仪及光学显微镜检测尿液白细胞的一致性。方法随机抽取2017年5～12月于我院住院患者的新鲜尿液标本500例,分别用Sysmex UF1000i全自动尿沉渣分析仪、Arkray AX-4030尿干化学分析仪及OLUMPUS CX31光学显微镜进行检测,对比分析3种检测方法检测结果是否一致。结果以显微镜检查作为对照,Sysmex UF1000i全自动尿沉渣分析仪和Arkray AX-4030尿干化学分析仪检测白细胞的敏感度分别为98.35%、71.90%,特异性分别为93.40%、85.75%,假阳性率分别为6.60%、1.58%,假阴性率分别为1.65%、67.77%,差异均有统计学意义(P<0.05)。结论在临床应用过程中,需要将3种方法联合检测。根据仪器检测规律制定镜检复检规则,才能快速、准确地为临床提供有效、可靠的检验结果。Objective To compare and analyze the consistency of urine white blood cells by Sysmex UF1000i automatic urine sediment analyzer, Arkray AX-4030 urine dry chemical analyzer and optical microscope. Methods A total of 500 fresh urine specimens from May to December 2017 in our hospital were randomly selected and detected by Sysmex UF1000i automatic urine sediment analyzer, Arkray AX-4030 urine dry chemical analyzer and OLUMPUS CX31 optical microscope. Whether the test results were consistent of the three test methods were compared and analyzed. Results Using microscopy as a control, the sensitivity of white blood cells test of the Sysmex UF1000i automatic urine sediment analyzer and the Arkray AX-4030 urine dry chemical analyzer was 98.35% and 71.90%, respectively, and the specificity was 93.40% and 85.75%, respectively; the false positive rates were 6.60% and 1.58%, respectively, and the false negative rates were 1.65% and 67.77%, respectively, and the differences were statistically significant （P〈0.05）. Conclusion In the clinical application process, the three detection methods need to be combined. According to the instrument detection rules, the microscopic examination and reexamination rules can be formulated to provide effective and reliable test results for the clinic quickly and accurately.
