摘  要:目的系统评价糖尿病患者中寄生虫的感染情况。方法电子检索PubMed、Medline、Embase、Cochrane Library、万方、中国期刊全文数据库,查找与糖尿病和寄生虫感染相关的横断面研究,检索时间为建库至2017年9月。检索纳入文献的参考文献。用RevMan 5.3软件进行Meta分析。结果最终纳入以糖尿病患者为研究对象的8篇文献,4091例患者,其中糖尿病组1 144例,对照组2 947例。结果显示:糖尿病组总体寄生虫感染率与对照组差异无统计学意义[比值比(OR)0.98;95%可信区间(CI)0.80,1.21];部分寄生虫感染在两组人群中差异有统计学意义,糖尿病发生丝虫感染的概率显著减低(OR 0.41;95%CI 0.24,0.69),发生弓形虫(OR 2.67(95%CI 1.48,4.82)、螨虫(OR 2.93;95%CI1.40,6.14)及粪类圆线虫(OR 3.90;95%CI 1.08,14.13)感染的风险显著增高。结论糖尿病不会增加寄生虫感染发生风险,但不同寄生虫感染情况存在差异,如糖尿病可使弓形虫、粪类圆线虫、螨虫感染发生的风险增加,使丝虫感染发生风险减低。Objective To systematically evaluate the effect of diabetes on the parasite infection.Methods PubMed, Medline, Embase, Cochrane Library, Wanfang, CAJ Full-text Database were retrieved electronically, and the cross-sectional study between the diabetes and parasite infection were searched, and the retrieval time was from the establishment of database to September 2017, and the Meta analysis was conducted by the RevMan 5.3 software. Results 8 papers and 4 091 cases of patients were finally included, including 1 144 cases in the diabetes group and 2 947 cases in the control group.s The dif- ferences in the total parasite infection rate between the diabetes group and the control group were not statistically significant (OR 0.98; 95% credibility interval CI 0.80, 1.21), and the difference in the partial parasitic infection between the two groups was obvious, and the occurrence probability of filarial infection obviously decreased (OR 0.41 ;95%CI 0.24, 0.69), and the occurrence risks of toxoplasma gondii (OR 2.67;95%CI 1.48, 4.82), mites (OR 2.93 ;95%CI 1.40, 6.14) and faecal roundworm (OR 3.90;95%CI 1.08, 14.13) obviously increased. Conclusion The diabetes can not increase the occurrence risk of parasite infection, but there are differences in different parasite infections, for example, the diabetes can increase the occurrence risks of toxoplasma gondii, toxoplasma faecalis and mites infection and decrease the occurrence risk of parasite infection.
