摘 要:目的观察基层神经外科应用显微手术治疗脑前循环动脉瘤的效果,为基层医院应用提供参考。方法方便选择该院2016年1月—2017年12月接收的脑前循环动脉瘤患者96例,均给予显微手术治疗,依照入院后手术开展时间分组,A组手术于入院后1~3 d开展,共33例,B组手术于入院后3~8 d开展,共42例,C组手术于入院后8 d后开展,共21例,观察治疗效果及并发症发生情况。结果经治疗后,A组优良率97.0%,B组优良率76.2%,C组优良率47.6%,A高于B组、C组,且B组高于C组,差异有统计学意义(P<0.05);A组、B组、C组术后并发症发生率分别为9.1%、11.9%、14.3%,3组差异无统计学意义(P>0.05)。结论基层神经外科应用显微手术治疗脑前循环动脉瘤患者时,具有较好的治疗效果,可减少患者术后并发症,但越晚开展手术时,治疗效果越差,因此应在确诊后尽早实施手术治疗,以提升治疗效果,改善患者预后。Objective To observe the effect of development of microsurgery for anterior circulation aneurysms in the department of primary neurosurgery thus providing reference for the primary hospitals. Methods 96 cases of patients with anterior circulation aneurysms admitted and treated in our hospital from January 2016 to December 2017 were convenient selected and treated with microsurgery, and the patients were divided into groups according to the operation after admission, and the group A with 33 cases developed the microsurgery in 1-3 d after admission, while the group B with 42 cases developed the microsurgery in 3-8 d after admission, and the group C developed the microsurgery in 8 d after admission, with 21 cases, and the treatment effect and occurrence of complications were observed. Results After treatment, the excellent and good rate in the group A, in the group B and in the group C was respectively 97.0%, 76.2%, 47.6%, and the indicator in the group A was higher than that in the group B and in the group C, and the indicator in the group B was higher than that in the group C, and the difference was obvious(P〈0.05), and the incidence rate of complications after surgery in the group A, group B and group C was respectively 9.1%, 11.9%, 14.3%, and the difference between the three groups was not obvious(P〉0.05). Conclusion The application of microsurgery for anterior circulation aneurysms in the department of primary neurosurgery has a better treatment effect, which can reduce the postoperative complications, and the later the development of the operation, the worse the treatment effect, therefore, we should implement the operation after confirmation, to improve the treatment effect and improve the prognosis.
