中西医结合治疗肝肾阴虚型重症IgA肾病临床研究

摘　　要：目的：探讨中西医结合方案治疗肝肾阴虚型重症IgA肾病患者的有效性及安全性。方法：24例肝肾阴虚型重症IgA肾病患者随机分为治疗组（中药和西药治疗）和对照组（中药模拟剂和西药治疗）,治疗后的每个月（即4周）检测24 h尿蛋白定量,观察中医证候指标,每2个月（即8周）空腹静脉采血,检测血常规、肝肾功能指标、血钾,进行疗效评价。结果：治疗8周、24周后,治疗组患者的24 h尿蛋白定量较治疗前相比,有显著下降（P〈0.05）;治疗24周后,治疗组的e GFR较治疗前相比,有显著升高（P〈0.05）;治疗16周、24周后,治疗组患者的中医主症积分较治疗前相比,有显著下降（P〈0.05）,与同期对照组相比,治疗组患者的中医主症积分有显著下降（P〈0.05）。治疗组的中医和西医治疗总有效率分别为91.67%、83.33%,均高于对照组的治疗总有效率16.67%、33.33%（P〈0.05）。结论：中西医结合治疗优于单纯西医治疗,具有降低患者24 h尿蛋白定量,改善患者肾功能的作用,安全、有效,可以更好的提高患者的生存质量。Objective：To explore the effectiveness and safety of the combination of traditional Chinese and Western medicine in the treatment of severe IgA nephropathy with Yin deficiency of liver and kidney type. Methods：24 cases of severe IgA nephropathy with Yin deficiency of liver and kidney type were randomly divided into the treatment group（traditional Chinese medicine and Western medicine）and the control group（Chinese medicine analogue and Western medicine）. 24 hours urinary protein quantitation was detected every month（4 weeks）after treatment,and TCM syndrome index was observed. Fasting venous blood was collected every 2 months（8 weeks）,blood routine test,liver and kidney function index and blood potassium were detected,and the curative effect was evaluated. Results：After 8 weeks and 24 weeks,the 24 hours urine protein of the patients in the treatment group was significantly lower than that before the treatment（P〈0.05）. After 24 weeks of treatment,the e GFR in the treatment group was significantly higher than that before the treatment（P〈0.05）. After treatment for 16 weeks and 24 weeks,the main symptom score of the treatment group was significantly lower than that before treatment（P〈0.05）. Compared with the control group,the main symptom score of the treatment group was significantly lower than that of the control group（P〈0.05）. The total effective rate of traditional Chinese medicine and Western medicine in the treatment group was 91.67% and 83.33% respectively,higher than that of the control group（16.67% and 33.33%,P〈0.05）. Conclusion：Integrated traditional Chinese and Western medicine treatment is better than Western medicine alone. It has the effect of reducing the 24 hours urine protein and improving the renal function of patients. It is safe and effective,and it can better improve the quality of life of patients.
