自拟方剂足痛宁浴足对糖尿病周围神经病变患者神经传导速度的影响

摘　　要：目的：探讨自拟方剂足痛宁浴足对糖尿病周围神经病变（DPN）患者神经传导速度的影响。方法：选取2015年3月—2017年5月本院收治的DPN患者102例,随机分为空白对照组、温水对照组及足痛宁组,各34例。3组均给予西医常规治疗,温水对照组另给予温水足浴,足痛宁组另给予中药足痛宁足浴。对患者治疗前后的临床症状变化情况进行评价,并测定治疗前后正中神经、腓总神经的运动传导速度（MCV）及感觉传导速度（SCV）。结果：治疗后疼痛数字分级法（NRS）评分、麻木评分及针刺感评分组间比较均有显著性差异（P 〈0. 05）,且足痛宁组治疗后NRS、麻木及针刺感评分均明显低于治疗前、空白对照组及温水对照组（P 〈0. 05）;治疗后正中神经、腓总神经的MCV及SCV组间比较有显著性差异（P 〈0. 05）,3组治疗后正中神经、腓总神经的MCV及SCV均明显快于治疗前（P 〈0. 05）,但温水组治疗后明显快于对照组（P 〈0. 05）,且足痛宁组治疗后明显快于对照组及温水组（P 〈0. 05）。结论：足痛宁浴足可加快神经传导速度,对DPN有一定治疗效果,值得推广。Objective： To explore the influence of self-made prescription of Zutongning feet bath on the nerve conduction velocity of patients with diabetic peripheral neuropathy（DPN）. Methods： A total of 102 patients with DPN were selected from March2015 to May 2017 in our hospital,and were randomly divided into blank control group,warm water control group and Zutongning group,and there were 34 cases in each group. The three groups were given routine treatment of western medicine. In addition,the warm water group was given warm water feet bath,and the Zutongning group was given Zutongning feet bath. The clinical neuropathy score of Toronto（TCSS） was used to evaluate the changes of clinical symptoms of patients before and after treatment,and the motor conduction velocity（MCV） and sensory conduction velocity（SCV） of the median nerve and the peroneal nerve were measured before and after treatment. Results： There were significant differences of the scores of digital grading（NRS）,numbness and acanthesthesia after treatment among the three groups（P〈0.05）,but the scores of NRS,numbness and acanthesthesia of the Zutongning group after treatment were significantly lower than those before treatment and those of the blank control group and the warm water control group（P〈0.05）. There were significant differences of MCV and SCV of the median nerve and common peroneal nerve after treatment among the three groups（P〈0.05）,and the MCV and SCV of the median nerve and common peroneal nerve of the three groups after treatment were significantly faster than those before treatment（P〈0.05）. The warm water group＇s after treatment were significantly faster than those of the control group（P〈0.05）. The Zutongning group＇ s after treatment were significantly faster than those of the control group and the warm water group（P〈0.05）. Conclusion： Zutongning feet bath can accelerate the nerve conduction velocity,with certain therapeutic effect on DPN,worthy of promoting.
