摘 要:目的运用三维可视化技术测定中国正常成人的脾脏体积。方法本研究收集来自上海全景医学影像中心200例正常体检者(男性121例,女性79例)的薄层CT图像及原始DICOM数据,并应用三维可视化技术进行三维成像,测定脾脏体积。结果 (1)正常成人脾脏体积为(185. 1±87. 43) ml,正常男性的脾脏体积为(201. 37±96. 37) ml,正常女性的脾脏体积为(160. 19±64. 62) ml,男性平均脾脏体积大于女性平均脾脏体积(P <0. 01);(2)成年男性93. 38%的脾脏体积分布在50~350 ml,成年女性约87. 34%的脾脏体积分布在50~250 ml;(3)传统公式计算的脾脏体积误差为-15. 76%(-50. 69%,44. 26%),其计算结果在总体、男性、女性中均与三维可视化结果存在差异(P <0. 01);(4)脾脏长径可解释62. 4%的脾脏体积变异,不能满足精准医疗的要求。结论 (1)采用三维可视化技术精准测定的正常脾脏体积可作为临床上成人脾脏体积的参考;(2)传统体积公式法误差大,脾脏二维参数不能精准反应脾脏体积,应采用三维可视化技术测量脾脏体积取代以往的二维脾脏评估法。Objective To determine the spleen volume (SV) of normal Chinese adult using three-dimensional (3D) visualization technology. Methods This study collected thin-slice computed tomography(CT) images and original DICOM data of physical examinations for 200 normal adults( 121 males and 79 females) from Shanghai Universal Medical Imaging Center. 3D visualization technology was used to determine SV. Results ( 1 ) The normal adult SV was ( 185. 1 ± 87. 43 ) ml, the normal male SV was (201.37 ±96. 37) ml, and the normal female SV was ( 160. 19 ± 64.62) ml. The average male SV was larger than the female average SV ( P 〈 0. 01) ; (2) 93.38% of male SV was distributed between 50 -350 ml. 87. 34% of female SV was distributed between 50 -250 ml; (3) The deviation of SV calculated by traditional formula was - 15.76% ( -50. 69 %, 44. 26% ) comparing to the 3D technology. The results were different from the 3D visualization results in the overall, male and female groups( P 〈 0.01 ) ; (4) The long diameter of the spleen could explain 62. 4% of the SV variation, which could not meet the requirements of precise medical treatment. Conclusions ( 1 ) The normal SV accurately measured by 3D visualization technology can be used as a reference for the adult SV; (2) The traditional volume formula has large deviation. Two - dimensional parameters of spleen can not accurately reflect SV. Spleen should be measured by 3D visualization technology. Volume assessment should replace the previous two-dimensional spleen assessment.
