腹腔镜胆囊切除术治疗Ⅰ型Mirizzi综合征

摘　　要：目的探讨"腹腔镜十字切开胆囊切除术"对Mirizzi综合征I型的治疗及疗效评价。方法回顾性分析2013年01月至2017年06月在我院应用"腹腔镜十字切开胆囊切除术"治疗的25例Mirizzi综合征I型患者的临床及影像资料。结果腹腔镜十字切开胆囊切除术成功23例,中转开腹2例,所有病例均痊愈出院,术后随访复查无一例胆道损伤、胆道狭窄和结石残留。结论通过对术前MRI精准评估为Mirizzi综合征Ⅰ型患者施行腹腔镜十字切开胆囊切除术,可避免医源性胆道损伤,是完全可行的。Objective To evaluate the method and effect of ＂Cross incision in laparoscopic cholecystectomy＂ by Mirizzi syndrome type Ⅰ. Methods 25 patients with Mirizzi syndrome type Ⅰ operated by ＂Cross incision in laparoscopic cholecystectomy＂ in 2013.01 - 2017.06 in the 3rd hospital of Nantong were retrospectively analyzed. Results Of the 25 patients, 23 were cured by ＂Cross incision in laparoscopic cholecystectomy＂ , 2 were converted to laparotomy and cured. There were no severe complications related to the operation such as extrahepatic bile duct injury. Conclusion The ＂Cross incision in laparoscopic cholecystectomy＂ for patients with Mirizzi syndrome type Ⅰ which were evaluated by MRI is feasible , the iatrogenic bile duct injury was avoid.
