摘 要:目的探讨指屈肌腱腱鞘炎患者应用滑膜内注射倍他米松治疗的临床效果。方法指屈肌腱腱鞘炎患者46例,均于滑膜内注射复方倍他米松注射液0.25mL,注射前和注射第31天行高频超声检查,观察肌腱横截面直径、厚度、横截面积及肌腱滑车厚度,并记录腱鞘炎分级变化。结果注射第31天,患者临床症状均改善,腱鞘炎分级均降低;肌腱横截面直径[(5.93±0.20)mm]、肌腱横截面积[(19.01±1.15)mm2]、肌腱滑车厚度[(0.70±0.05)mm]均较注射前[(6.29±0.20)mm、(21.67±1.34)mm2、(1.12±0.10)mm]减小(P<0.05),肌腱厚度[(4.38±0.12)mm]与注射前[(4.52±0.16)mm]比较差异无统计学意义(P>0.05);注射后均未出现严重不良反应。结论指屈肌腱腱鞘炎患者滑膜内注射倍他米松可降低肌腱滑车厚度,缩小肌腱横截面积,缓解临床症状,安全性好。Objective To investigate the effect of intrasynovial injection of betamethasone on tenosynovitis of the flexor tendon.Methods Forty-six patients with tenosynovitis of the flexor tendon received intrasynovial injection of 0.25 mL betamethasone,and received high-frequency ultrasound examination before and on the 31 st day after injection to observe the transverse diameter,thickness and cross-sectional area of the tendon as well as the thickness of the pulley,and the tenosynovitis degree was recorded.Results On the 31 st day after injection,the clinical symptoms were markedly relieved,and the tenosynovitis degree was reduced.The transverse diameter((5.93±0.20)mm),cross-sectional area((19.01±1.15)mm2)and thickness of the pulley((0.70±0.05)mm)reduced obviously on the 31 st day after injection in comparison with those before injection((6.29±0.20)mm,(21.67±1.34)mm2,(1.12±0.10)mm)(P<0.05),while the thickness of the tendon showed no significant difference in comparison with that before injection((4.38±0.12)mmvs(4.52±0.16)mm)(P>0.05).Conclusion Intrasynovial injection of betamethasone could reduce the thickness and cross-sectional area of the tendon and pulley to relieve the clinical symptoms with good security in patients with tenosynovitis of the flexor tendon.
