益气活血汤联合西药治疗糖尿病下肢血管病变(气虚血瘀)随机平行对照研究

摘　　要：[目的]观察益气活血汤联合西药治疗糖尿病下肢血管病变(气虚血瘀)疗效。[方法]使用随机平行对照方法,将100例门诊患者按诊顺序号方法随机分为两组。对照组50例阿托伐他汀,20mg/次,1次/d,随病情变化加减,最多不超过80mg/d;前列地尔1～2mL+10mL生理盐水,1次/d,静注。治疗组50例益气活血汤(黄芪30g,人参12g,桃仁、红花、赤芍、牛膝各15g,当归尾、地龙各10g,水蛭6g),水煎400mL,1剂/d,2次/d,阿托伐他汀、前列地尔治疗同对照组。治疗3个月为1疗程。观测临床表现、踝肱指数(ABI)、总胆固醇、甘油三酯、不良反应。治疗1疗程(3个月),判定疗效。[结果]治疗组显效33例,有效13例,无效4例,总有效率92.00%;对照组显效28例,有效12例,无效10例,总有效率80.00%;治疗组疗效优于对照组(P<0.05)。踝肱指数(ABI)两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。总胆固醇、甘油三酯两组均有下降(P<0.05,P<0.01),治疗组下降多于对照组(P<0.01)。[结论]益气活血汤联合西药治疗糖尿病下肢血管病变(气虚血瘀),疗效满意,无严重不良反应,值得推广。[Objective] To observe the curative effect of Yiqi Huoxue decoction（益气活血汤） combined with atorvastatin on diabetic lower extremity angiopathy（Qixu Xueyu）. [Methods] 100 outpatients were randomly divided into two groups according to the order of diagnosis. In the control group, 50 cases were treated with atorvastatin, 20 mg/times, and 1 times/d; alprostadil,1～2 mL＋10 mL saline, 1 times/d, intravenous injection.Treatment group 50 cases of Yiqi Huoxue decoction（Huangqi 30 g, Renshen 12 g, Taoren,Honghua, Chishao,Niuxi each 15 g, Dangguiwei, Dilong each 10 g, Shuizhi 6 g）, decoct 400 mL, 1 Ji/d, 2 times/d; atorvastatin treatment with the same control group. Treatment for 3 months is 1 courses. Clinical symptoms, ankle brachial index（ABI） and adverse reactions were observed. 1 courses of treatment（3 months） were used to determine the curative effect. [Results] 33 cases were markedly effective, 13 cases were effective, 4 cases were ineffective,the total effective rate was 92.00%; 28 cases were markedly effective, 12 cases were effective, 10 cases were ineffective, the total effective rate was 80.00%; the curative effect of the treatment group was better than that of the control group（P〈0.05）. The ankle brachial index（ABI） was improved in the two groups（P〈0.01）, and the improvement in the treatment group was better than that in the control group（P〈0.01）. Total cholesterol and triglyceride decreased in both groups（P〈0.05, P〈0.01）, and the decrease in treatment group was more than that in control group（P〈0.01）. [Conclusion] Yiqi Huoxue Decoction combined with atorvastatin in the treatment of diabetic lower extremity angiopathy（Qi deficiency and blood stasis） has a satisfactory effect without serious adverse reactions and is worthy of promotion.
