医改后医联体抗菌药物使用情况调查与合理性分析

摘　　要：目的:了解北京市朝阳区南部医联体医改后抗菌药物临床应用情况,分析和评价其用药合理性。方法:回顾性调查以北京市垂杨柳医院为核心的朝阳区南部医联体14家社区卫生服务中心2017年5月至2018年4月期间抗菌药物处方3436张,对抗菌药物使用情况进行分析,并根据制定的抗菌药物合理性评估标准进行用药合理性评估,对处方问题进行分类整理。结果:该医联体常用抗菌药物的药物利用指数(DUI)范围为0.86～1.12;处方合理性评估发现不规范处方271张(7.89%),用药不适宜处方352张(10.24%),其中用法用量不适宜、遴选的药品不适宜为主要问题。结论:医改后该医联体在抗菌药物使用方面存在部分不合理问题,开展医联体抗菌药物使用情况进行监测与合理性分析,为合理用药干预管理及持续改进提供参考。ObjectiveTo explore the clinical application of the antibacterial drug after medical reform in the Southern Medical Treatment Combination of Chaoyang District, Beijing, and evaluate and analyze the rationality of the antibacterial drug.Methods3436 antibacterial drug prescriptions from May 2017 to April 2018 were extracted retrospectively and the utilization of antibacterial drug was investigated. According to the antibacterial drug evaluating standard, the medication rationality of antibacterial drug was evaluated, and the inappropriate cases were classified.ResultsThe drug utilization index （DUI） of the commonly used antibacterial drug in the medical treatment combination ranged from 0.86 to 1.12. Totally 271 （7.89%） undisciplined prescriptions and 352 （10.24%） inappropriate prescriptions were found according to the rationality evaluation, of which, irrational dosage, improper drug selection was the major problems.ConclusionThere were some unreasonable problems in the medical treatment association after the medical reform. Drug usage assessment and prescription review was implemented to identify typical problems and provide reference for rational drug intervention and continuous improvement.
