摘  要:目的 探讨缺氧诱导因子(hypoxia-inducible factor,HIF)诱导上皮钙粘蛋白(Epithelial cadherin,E-cad)胶原转化与逼尿肌功能的相关性.方法 选择2014年5月至2017年10月泌尿外科入院手术治疗的膀胱出口梗阻(bladder outlet obstruction,BOO)所致逼尿肌过度活动(OAAB)患者50例,设为观察组;选择同期入院治疗的膀胱出口梗阻无OAB患者50例,设为对照组.采用UDS-94-BT尿动力仪完成两组患者最大尿流率、逼尿肌不稳定(detrusor instability,DI)及最大尿流率逼尿肌压力.采用免疫组织化学法测定两组逼尿肌HIF、E-cad胶原蛋白表达;对膀胱出口梗阻HIF诱导Ecad胶原转化与逼尿肌功能进行相关性分析.结果 观察组HIF阳性率表达,均高于对照组(P<0.05);E-cad胶原蛋白表达低于对照组(P<0.05).观察组最大尿流率及最大尿流率逼尿肌压力,均小于对照组(P<0.05);观察组DI率,高于对照组(P<0.05);SPSS Pearson相关性分析结果表明:观察组最大尿流率、DI及最大尿流率逼尿肌压力与HIF胶原蛋白阳性率呈负相关性(P<0.05),而与E-cad呈正相关(P<0.05).结论 HIF与E-cad在膀胱出口梗阻患者中表达异常,且与逼尿肌功能明显相关,对膀胱出口梗阻逼尿肌功能异常发病机制的研究以及疾病的治疗提供了新的思路.Objective To investigate the correlation between hypoxia inducible factor (HIF) induced collagen transformation of epithelial cadherin (E-cad) and detrusor function.Methods From May 2014 to October 2017,50 patients with overactive bladder(OAB) caused by bladder outlet obstruction(BOO) undergoing urological surgery were selected as observation group,and 50 cases of bladder outlet obstruction without detrusor instability were selected as control group at the same time.The maximal uroflow rate and detrusor instability(DI) were performed in the two groups by UDS-94-BT urodynamic analyzer and detrusor pressure with maximal uroflow rate.Tissue samples were taken from the lesion and the expressions of HIF and E-cad in the two groups were measured by immunohistochemical method.SPSS Pearson correlation analysis was used to analyze the correlation between E-cad collagen conversion and detrusor function induced by bladder outlet obstruction HIF.Results The positive rate of HIF in the observation group was higher than that in the control group (P < 0.05),and the expression of E-cad collagen in the observation group was lower than that in the control group (P < 0.05).The maximal uroflow rate and maximal uroflow velocity in the observation group were lower than those in the control group (P < 0.05).DI rate in the observation group was higher than that in the control group (P < 0.05).SPSS Pearson correlation analysis showed that the positive rates of HIF,Ecad collagen in the observation group and the maximum uroflow rate,DI and maximum uroflow rate.Urinary muscle pressure was negatively correlated (P < 0.05).Conclusions The expression of HIF and E-cad is abnormal in the patients with bladder outlet obstruction,and is significantly related to detrusor function.It provides a new idea for the study of the abnormal mechanism of detrusor dysfunction and the treatment of the disease.
