隔药灸天枢、大肠俞治疗术后早期炎性肠梗阻  

Treatment of early postoperative inflammatory intestinal obstruction with medicine-separated moxibustion at Tianshu and Dachangshu

摘　　要：目的观察隔药灸天枢、大肠俞治疗术后早期炎性肠梗阻的临床疗效。方法将76例患者分为治疗组和对照组,各38例。对照组接受生长抑素治疗;治疗组给予天枢、大肠俞隔药灸治疗。治疗7 d后观察主要症状、肠功能情况、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平,评价临床疗效。结果治疗组临床疗效总有效率为92.11%优于对照组的78.95%(P <0.05)。治疗后,治疗组肛门恢复排气时间、腹部症状缓解时间、肠鸣音恢复时间少于对照组,差异有统计学意义(P <0.05)。治疗后2组血清CRP、TNF-α、IL-6比较差异有统计学意义(P <0.05)。结论隔药灸天枢、大肠俞能够明显改善术后早期炎性肠梗阻患者的临床症状,有效的调节血清CRP、TNF-α、IL-6水平,从而起到减少肠道炎性渗出,减轻肠道水肿的治疗作用。Objective To observe the clinical effect of herbal moxibustion on Tianshu and Dachang shu in the treatment of early postoperative inflammatory intestinal obstruction. Methods 76 patients were divided into treatment group and control group, 38 cases in each group. The control group was received somatostatin treatment and the treatment group was treated with herbal separation moxibustion on Tianshu and Dachangshu. The main symptoms, intestinal function, tumor necrosis factor-α（TNF-α） and interleukin-6（IL-6） levels of c-reactive protein（CRP） were observed 7 days after treatment. Results The total effective rates of the treatment group and the control group were 92.11% and 78.95% respectively（P〈0.05）. After treatment, anal exhaust time, abdominal symptoms time, recovery time of bowel sounds were shorter than the control group. The difference was statistically significant（P〈0.05）. There was significant difference in serum CRP, TNF-α, IL-6 between the two groups after treatment（P〈0.05）. Conclusion Herbal moxibustion at Tianshu and Dachangshu can obviously improve the clinical symptoms of patients with early postoperative inflammatory intestinal obstruction, and effectively regulate the levels of serum CRP, TNF-α, IL-6 in order to reduce intestinal inflammatory exudation and reduce intestinal edema.
