聚丙烯酰胺水凝胶注射隆乳患者术后乳腺组织CD68、CD25、PCNA表达分析

摘　　要：目的 分析聚丙烯酰胺水凝胶注射隆乳患者术后乳腺组织CD68、CD25、PCNA表达情况.方法 随机选取2015年6月至2016年12月贵阳市妇幼保健院接诊的有隆乳剂取出要求的聚丙烯酰胺水凝胶注射隆乳患者40例作为观察组,根据患者有无乳腺硬结又分为无硬结组15例、有硬结组25例,对两组患者分别进行苏木素-伊红（HE）染色;另选取同期就诊的未注射聚丙烯酰胺水凝胶的15例乳腺组织正常的女性作为对照组,分别应用免疫组化法检测各组乳腺组织中CD68、CD25、PCNA的表达情况,比较组间CD68、CD25、PCNA阳性细胞计数.结果 观察组患者乳腺组织切片经HE染色后,于光镜下可见聚丙烯酰胺水凝胶被大面积淡紫蓝染,其周边组织存在不同程度的纤维增生,且注射时间越长患者增生程度越严重,另可见小血管明显扩张,且存在炎性细胞、淋巴细胞及浆细胞.分析显示：观察组CD68、PCNA阳性表达情况（25/40、29/40）高于对照组（1/15、1/15）,差异有统计学意义（χ2=13.644、19.070,P均〈0.001）;其中有硬结组CD68、PCNA阳性表达情况（21/25、24/25）高于无硬结组（4/15、5/15）,差异有统计学意义（χ2=13.148、15.457,P均〈0.001）;有硬结组CD25阳性表达比例虽略高于无硬结组和对照组,但差异未见统计学意义（P〉0.05）.观察组CD68、PCNA阳性细胞计数均高于对照组,差异有统计学意义（t=59.773、29.494,P均〈0.001）;其中有硬结组CD68、PCNA阳性细胞计数均高于无硬结组,差异有统计学意义（t=10.951、22.702,P均〈0.001）;但有硬结组、无硬结组及对照组CD25阳性细胞计数比较,差异未见统计学意义（P〉0.05）.结论 采用聚丙烯酰胺水凝胶注射隆乳患者乳腺组织中的CD68、CD25、PCNA阳性细胞会明显增多,其在一定程度上会引起免疫应答反应,该免疫反应活跃程度不仅不会随着时间的推移而衰减,且在一定程度上会促进细胞的过度增Objective To analyze the expression of CD 68 , CD25 and PCNA in breast tissues of patients who experienced breast augmentation with polyacrylamide hydrogel injection .Methods A total of 40 patients, who experienced breast augmentation with polyacrylamide hydrogel injection first and then required to removal it in Guiyang Maternal and Child Health Hospital from June 2015 to December 2016, were randomly selected as observation group .According to morbidity of breast induration , the patients were divided into non-induration group （15 cases） and induration group （25 cases）.And hematoxylin-eosin （ HE ） staining were carried on patients in the two groups .Another 15 normal women without injecting polyacrylamide hydrogel were selected as control group .The expression of CD68, CD25 and PCNA in breast tissues were detected by immunohistochemistry , and the positive cell counts of CD 68, CD25 and PCNA were compared among these groups .Results After HE staining , polyacrylamide＆nbsp;hydrogel was stained by lavender blue in a large area , and fibrosis was found in the surrounding tissues of patients in observation group .The hyperplasia of the patients got worse with in increase in duration of injection.In addition, the dilatation of small blood vessels was obvious and inflammatory cells and lymphocytes could be found .Analysis results showed that： the positive rates of CD68 （ 25/40 ） and PCNA （29/40） in observation group were higher than those in control group （1/15, 1/15）, and the differences were statistically significant （χ2 =13.644 , 19.070 , both P〈0.001 ）; the positive rates of CD68 and PCNA in induration group （21/25, 24/25） were higher than those in non-induration group （4/15, 29/40）,the differences were ststistically singnificant （χ2 =13.148, 15.457;both P〈0.001）.
