摘  要:目的 探讨呼出气一氧化氮浓度(Fe NO)联合潮气肺功能在婴幼儿喘息中应用价值。方法 选取2016年6月-2017年6月台州市立医院和台州恩泽医疗中心(集团)恩泽医院儿科因婴幼儿喘息住院的患儿120例,根据哮喘预测指数分为API阳性组和API阴性组各60例,2组均按婴幼儿喘息进行常规处理,对2组治疗前Fe NO水平和潮气肺功能进行检测,并比较API阳性组不同喘息程度和临床疗效之间Fe NO水平和潮气肺功能变化。结果 婴幼儿喘息不同API分组,Fe NO水平API阳性组显著高于API阴性组,差异有统计学意义(t=20. 87,P 〈0. 01),而潮气肺功能TPEF/Te(%)、VPEF/Ve(%) API阳性组与API阴性组差异无统计学意义(P〉0. 05); Fe NO浓度和潮气肺功能TPEF/Te、VPEF/Ve与API阳性组喘息程度呈显著正相关(r值分别为0. 76、0. 69-0. 70,P 〈0. 01);不同临床疗效与Fe NO和潮气肺功能呈显著正相关(r值分别为0. 71、0. 66-0. 68,P 〈0. 01)。结论 Fe NO水平和潮气肺功能TPTEF/Te、VPTEF/Ve均可反映喘息程度和疗效,而Fe NO水平对婴幼儿哮喘诊断有一定参考价值。Objective To observe the application value of exhalation nitric oxide concentration combined with tidal air lung function in infant wheezing. Methods A total of 120 infant patients with wheezing who hospitalized at the department of pediatrics in Taizhou Municipal Hospital and Enze Hospital of Taizhou Enze Medical Center(Group) from June 2016 to June 2017, with 60 cases each in API positive group and API negative group, and according to the prediction index of asthma, both groups were routinely treated with infant wheezing. Then the FeNO level and tidal air lung function were detected before treatment, and which were compared with the different wheezing degree and clinical effect in the API positive group. Results In different API groups for infant with wheezing, the FeNO level in the API positive group was significantly higher than that in the API negative group ( t = 20.87, P 〈 0.01 ). However, there was no statistical significance on the differences of tidal air lung function[ TPEF/ Te ( % ) and VPEF/Ve( % )] between API positive group and API negative group ( P 〉 0.05 ). The level of FeNO and the tid- al air lung function of TPEF/Te( % ) and VPEF/Ve( % ) were positively correlated with wheezing degree in API positive group, with the r value of 0.76 and within 0.69 - 0.70, respectively ( P 〈 0.01 ). The correlation between the different clinical cura- tive effect and the level of FeNO and tidal air lung function was positively correlated, with the r value of 0.71 and within 0.66 - 0.68, respectively ( P 〈0.01 ). Conclusion The FeNO level and tidal air lung function TPTEF/Te, VPTEF/Ve can all reflect the degree of infant wheezin- and efficacy, while FeNO level has a certain reference value to the diagnosis of infant asthma.
