摘  要:目的:探究动态增强磁共振成像(DCE-MRI)定量分析宫颈鳞癌微血管通透性的价值。方法:选取我院2016年4月至2017年9月收治的宫颈鳞癌患者36例(Ⅱ期18例、Ⅲ期18例),并选择同时期收治的宫颈正常患者(18例)为研究对象。分别采用DCE-MRI扫描进行检查及定量分析。观察肿瘤组织的TIC曲线特性并对比宫颈鳞癌组与正常宫颈组的微血管参数值和宫颈鳞癌Ⅱ期组和宫颈鳞癌Ⅲ期组的微血管参数值。结果:宫颈癌组织的TIC曲线为"Ⅲ"型。对照组宫颈正常的患者Ktrans、Ve及Kep明显低于宫颈鳞癌组;宫颈鳞癌Ⅱ期组Kep值显著低于宫颈鳞癌Ⅲ期组,Ve值显著高于宫颈鳞癌Ⅲ期组(P<0.05),2组Ktrans值比较差异无统计学意义(P>0.05)。结论:DCE-MRI定量分析宫颈鳞癌微血管通透性具有较高的价值。Objective: To investigate the value of quantitative dynamic contrast-enhanced MRI (DCE-MRI) in assessment of microvascular permeability of cervical squamous cell carcinoma. Methods: Thirty-six patients with cervical squamous cell carcinoma from Apt 2016 to Sep 2017 were enrolled and were divided into stage Ⅱ group (18 cases) and stage In group (18 cases) , and 18 cases of patients with normal cervix were sellected as control group. The three groups were examined using DCE-MRI. The time- intensity curve (TIC) was drawn. The microvascular parameters K^trars, Ve and Kep of the 3 groups were compared. Results: The TIC curve of cervical squamous cell carcinoma was type Ⅲ. K^trans, Vo and Kep values in cervical squamous cell carcinoma group were significantly higher than those in control group (P〈0.05). Kep value in stage Ⅱ group was significantly lower than that in stage Ⅲ group, while Vo value was significantly higher (P〈0.05). The difference in K^trans value between the two groups was not statistically significant (P〉0.05). Conclusion: Quantitative analysis of DCE-MRI can be used to assess microvasculty permeability of cervical squamous cell carcinoma.
