摘 要:目的探讨慢性阻塞性肺疾病(COPD)患者外周血和肺组织中高迁移率族蛋白B1(high mobility group box 1,HMGB1)及Toll样受体4(Toll like receptor 4,TLR4)的表达水平及意义。方法检测并对比104例COPD患者[急性加重期(AE-COPD)组56例、稳定期(ST-COPD)组48例]与40例健康受试者(对照组)的肺功能指标、外周血及肺组织中HMGB1及TLR4的表达差异,并对肺功能参与二者表达水平进行相关性分析。结果 COPD患者组FEV1%FVC、FEV1%pred均显著高于对照组(P <0. 05),3组比较,差异均有统计学意义(P <0. 05); COPD患者外周血及肺组织中HMGB1及TLR4的表达水平均显著高于对照组(P <0. 05),且随着COPD病情加重,外周血及肺组织中HMGB1及TLR4的表达水平逐渐上调; COPD患者外周血及肺组织中HMGB1及TLR4的表达水平与肺功能参数均呈显著负相关(|r|均> 0. 5,P <0. 05)。结论 HMGB1及TLR4可能通过表达上调共同调控COPD的发病机制,为临床COPD的预防和针对性治疗的参考指标。Objective To explore the expression levels and significance of high mobiliU group boxl ( HMGB1 ) and toll like receptor 4 ( TLR4 ) in peripheral blood and lung tissue of patients with chronic obstructive pulmonary disease (COPD). Methods A total of 104 patients with COPD were divided into acute exacerbation COPD group (AE-COPD group, n =56) and stable period group (ST-COPD group,n = 48 ), at the same time, the other 40 healthy subjects who recived physical examination in our hospital were enrolled as control group. The lung function indexes, the differences of expression levels of HMGB1 and TLR4 in peripheral blood and lung tissue were detected and compared among the three groups. Moreover the correlation between pulmonary function and the expression levels of HMGB1, TLR4 was analyzed. Results The levels of forced expiratory- volume in forced vital capacity (FEV1% FVC) and forced expiratory volume in 1 s predicted value (FEV1% pred) in COPD group were significantly higher than those in control group (P 〈 0.05 ) , and there were significant differences among the three groups(P 〈 0.05). The expression levels of HMGB1 and TLR4 in peripheral blood and lung tissues in COPD group were significantly higher than those in control group ( P 〈 0.05 ). Moreover the expression levels of HMGB1 and TLR4 in peripheral blood and lung tissues were gradually increased along with the aggravation of the COPD. The expression levels of HMGB1 and TLR4 in peripheral blood and lung tissues were closely negatively correlated with pulmonary function indexes of patients with COPD ( P 〈 0.05 ). Conclusion The proteins of HMGB1 and TLR4 may upregulate the expression levels to regulate the pathogenesis of COPD. This research provides reference indexes for prevention and targeted treatment of COPD.
