摘 要:目的观察肾损伤分子-1(Kim-1)、分泌型卷曲相关蛋白5(SFRP-5)和生长分化因子-15(GDF-15)在糖尿病肾病早期诊断中的价值。方法选择2015年1月至2017年6月就诊的2型糖尿病患者166例,均符合2型糖尿病的诊断,并根据是否合并肾病,分为单纯糖尿病组65例和糖尿病肾病组101例。选择同期行健康体检患者30例为对照组。采用酶联免疫吸附试验检测Kim-1、GDF-15、SFRP-5和HbA1c。观察单纯糖尿病组,糖尿病肾病组和健康对照组血清Kim-1、GDF-15和SFRP-5水平的变化,及糖尿病肾病患者血清Kim-1、GDF-15和SFRP-5水平与肾病严重程度和HbA1c的相关性,及各指标之间的相关性分析。结果糖尿病肾病组的血清Kim-1和GDF-15水平明显高于2型糖尿病组和对照组(P <0. 01),而2型糖尿病组明显高于对照组(P <0. 01);糖尿病肾病血清SFRP-5水平明显低于2型糖尿病组和对照组(P <0. 01),而2型糖尿病组明显低于健康对照组(P <0. 01)。糖尿病肾病血清Kim-1和GDF-15水平随着肾功能严重程度和HbA1c水平的升高而升高(P <0. 01),而SFRP-5水平出现明显降低。糖尿病肾病患者血清Kim-1水平与GDF-15呈相关(r=0. 796,P <0. 05),与SFRP-5水平呈负相关(r=0. 687,P <0. 05),GDF-15水平与SFRP-5水平呈负相关(r=0. 825,P <0. 05)。结论 Kim-1、GDF-15和SFRP-5参与了糖尿病肾病发生发展,是判断糖尿病肾病严重程度的重要指标,具有重要的临床价值。Objective To observe the value of kidney injury molecule-1 (Kim-1), secreted frizzled-related protein-5 ( SFRP-5 ) and growth differentiation factor-15 ( GDF-15 ) in early diagnosis of diabetic nephropathy. Methods A total of 166 patients with Upe 2 diabetes mellitus, who were diagnosed and treated in our hospital from January 2015 to June 2017, were divided into simple diabetes mellitus group (n = 65) and diabetic nephropathy (DN) group (n = 101 ) according to the diagnosis of nephropathy. Meanwhile, 30 healthy subjects who underwent physical examination were enrolled as control group. Enzyme linked immunosorbent assay was used to detect the serum levels of Kiml, GDF15, SFRP5 and HbAlc. The changes of the serum levels of Kiml, GDF15 and SFRP5 in three groups and the correlation between the serum levels and the severiU of nephropathy were observed and compared. In addition, the correlation between the indicators was analyzed. Results The serum levels of Kiml and GDFI5 of the patients in DN group were significantly higher than those in simple diabetes mellitus group and control group ( P 〈 0.01 ). And those in simple diabetes mellitus group were significantly higher than those in control group (P 〈 0.01 ). The serum levels of SFRP5 in DN group were significantly lower than those in simple diabetes mellitus group and control group ( P 〈 0.01 ). And those in simple diabetes mellitus group were significantly lower than those in control group (P 〈 0.01 ). Moreover, the serum levels of Kiml and GDF15 were increased and the levels of SFRP5 were decreased as the severity of renal function and HbAlc levels were increased (P 〈 0.01 ). In addition, the serum levels of Kiml of the patients in DN group were positively correlated with the levels of GDF15 (r = 0. 796 ,P 〈 0.05 ) and negatively correlated with SFRP5 (r = 0. 687, P 〈 0.05).
