两种细胞外基质分子在慢性肾衰中的变化

摘　　要：目的 :检测慢性肾衰 (CRF)患者血清透明质酸 (HA)及层粘蛋白 (LN)的含量 (x±s,μg L) ,并探讨它们在CRF病理过程中的作用。方法 :用放射免疫法检测 31例CRF患者及 4 0例健康人血清HA及LN浓度 ,统计分析用t检验。结果 :CRF血清HA和LN均比正常对照组显著升高 (P <0 0 1)。其中HA在少尿或无尿期CRF中高于正常对照 5～ 10倍。 11例CRF患者经血透析治疗后血清HA与LN均明显下降 (P <0 0 1)。结论 :HA和LN是构成ECM的重要成分 ,其含量异常增多是致ECM分子过度沉积 ,进而导致肾纤维化及肾功衰竭的因素之一。血液透析治疗可清除或减少ECM的过度蓄积并有助于改善肾功能 ,提示可通过抗ECM或刺激基质分解等途径 。Objective:To detect the contents of hyaluronic acid(HA),Laminin(LN) and probe their role in the pathologic course of chronic renal failure(CRF).Methods:The serum HA and LN from 37 patients with CRF and 40 healthy adults were measured by RIA.T test was used in data(x±s) analysis.Results:The HA and LN in CRF serum evidently increased than those of normal controls,respectively(P<0.01).The contents of HA and LN (x±s) were higher 5～10 time than normal control in oliguria or anuria stages.After hemadialysis,the contents of HA and LN obviously decreased than that of before(P<0.01).Conclusion:HA and LN were two important components of extracellular matrix(ECM) which exessive producuton accelerate ECM deposition and participate the renal in jury of CRF.The hemedialysis therapy can eliminate the HA and LN and relieve the patients condition with CRF.Anti ECM or stimulatiny the degradation of ECM would be a promise method for the CRF therapy in the future.
