摘　　要：目的　介绍应用拇指桡掌侧动脉为蒂的大鱼际逆行岛状皮瓣修复拇指指腹缺损的手术方法。方法　1 996年至 2 0 0 1年 ,共修复拇指指腹缺损 1 5例。皮瓣设计在大鱼际区 ,以拇指桡掌侧动脉为蒂 ,旋转点在指间关节以近。皮瓣面积最小为 1 5cm× 3 0cm ,最大为 3 0cm× 3 .5cm。结果　所有皮瓣全部成活。术后随访半年到 1年 ,指腹外观 ,皮肤弹性、色泽均良好。指腹二点分辨觉为 8～ 1 0mm。结论　拇指桡掌侧动脉逆行岛状皮瓣操作简单、血供可靠 ,是修复拇指指腹软组织缺损的较好方法。Objective To introduce a new surgical procedure for repair of thumb pulp defect with greater thenar reversed island flap pedicled with thumb radial palmar artery. Methods 15 cases of thumb pulp defects were repaired from 1996 to 2001. The flap was devised at the greater thenar with pedicle of thumb radial palmar artery. The rotation point was proximal to the interphalangeal joint. The flap ranged from 1.5 cm × 3.0 cm to 3.0 cm × 3.5 cm. Results All 15 flaps survived. The appearance, elasticity and color of the flap were obtained satisfactorily at 6 to 12 months postoperative follow up, with 2 PD 8 to 10 mm.Conclusions The greater thenar reversed island flap pedicled by thumb radial palmar artery is a good option for repair of the thumb soft tissue defect and has advantages of simple performance and reliable blood supply.
