摘  要:目的 :探讨胸水DNA倍体及细胞周期检测对胸水良恶性的诊断价值。方法 :通过流式细胞仪 (FCM)对 114例胸腔积液患者进行胸水DNA含量的测定以及细胞增殖状态分析 (G0 /G1、S、G2 /M期 )。结果 :①良性胸水DI =1;②癌性胸水DI=1.30~ 3.81,均值为 2 .30 ,癌性胸水DI指数显著高于良性胸水 ;③DNA细胞周期分析 :胸水性质G0 /G1期、S、G2 /M期(% )值在良性胸水时分别为 94 .3± 0 .3、2 .4± 0 .7、3.0± 0 .9,在恶性胸水时分别为 72 .5± 0 .6、11.9± 3.0、16 .2± 4 .2。两组细胞增殖比值有非常显著性差异 (P <0 .0 1) ;④癌性胸水经过一次全身化疗及胸腔内注入化疗药物 1周后测DI有所下降 (均值为 1.6 7)。结论 :对胸水细胞学难以诊断时 。Objective: To investigate the detection of pleural fluid DNA diploid and cell cycle in diagnosing pleural fluid types. Methods: Flow cytometer was used to detect pleural fluid DNA content and to analyze cell proliferation in 114 cases with hydrothorax. Results: ①DI=1 in benign hydrothorax; ②DI=1.30~3.81 (mean value was 2.30) in malignant hydrothorax. DI in malignant hydrothorax was significantly higher than that in benign hydrothorax; ③Analysis of the DNA cell cycle: in benign hydrothorax, the values of G 0/G 1 phase, S phase, G 2/M phase(%) were 94.3±0.3, 2.4±0.7, 3.0±0.9, respectively, and in malignant hydrothorax, those were 72.5±0.6, 11.9±3.0, 16.2±4.2 respectively. There was significant difference in cell proliferation in the two groups (P<0.01); ④In cancer, DI to some extent were decreased after chemotherapy. Conclusion: This checkout is significant when it is hard to diagnose cytology in pleural fluid.
