摘 要:目的 了解二尖瓣狭窄下游跨瓣血流流场中不同位点湍流剪应力 (turbulentshearstress,TSS)大小与流场均匀性和狭窄程度间的关系。方法 采用多普勒超声结合计算机图像分析技术 ,对一组不同程度二尖瓣狭窄患者二尖瓣下游不同位点的TSS与流场均匀性进行定量检测。结果 边界位点TSS、流场均匀性以及有效瓣口面积在不同狭窄程度组间均存在差异显著性 (P <0 0 5 ) ,且相互间明显相关 (r分别为 0 81~ 0 84和 0 82~ - 0 88)。结论 二尖瓣狭窄下游TSS集中分布于射流边界湍流区 ,狭窄越重 ,流场均匀性越差 ,TSS越大。TSS的研究有助于深入认识瓣膜病发生发展规律和探讨更为有效的治疗方法。Objective To study the relationship between the magnitude of turbulent shear stress (TSS)on different locations and the flow field uniformity downstream of mitral stenosis as well as the stenotic extent Method Using color Doppler echocardiography combined with the analysis of image by computer, we measured quantitatively TSS and flow field uniformity on different locations downstream of mitral valve in a group of patients with different degree of mitral stenosis Results A significant difference was found in TSS on the boundary locations, flow field uniformity and effective orifice area among the groups with different degree of stenosis( P <0 05) These items correlated with each other ( r = 0 81-0 84 and -0 82--0 88 respectively) Conclusion TSS downstream of mitral stenosis focused in the area of turbulence in boundary of jet The more severe stenosis, the worse flow field uniformity and the bigger TSS The study for TSS would be helpful to reveal the prognosis of valvular disease and to find more effective treatment for it
