改良式子宫三角切除术的临床研究

摘　　要：目的:探讨改良式子宫三角切除术的临床应用价值。方法:符合条件的患者82例,随机分成改良式子宫三角切除术组(研究组)和常规子宫全切术组(对照组),观察两组手术各项指标及手术对卵巢功能、性生活质量的影响。结果:血清FSH、P及E2研究组术后与术前比较,差异无显著性(P>0.05),对照术后与术前比较,差异有显著性(P<0.05)。两组术后恢复排气及排尿时间、性生活满意度、更年期症状的发生率差异均有显著性(P<0.05)。结论:改良式子宫三角切除术对卵巢功能影响小,不降低性生活满意度,对大小便无明显影响,切除了宫颈癌的好发部位,防止术后阴道残端及输卵管脱垂和残留卵巢综合征等并发症的发生,有临床应用价值。Objective:To investigate clinical value of modified triangle hysterectomy.Methods:Eighty-two pre-menopausal women with benign uterine disease who underwent hysterectomy were included in this study.Among them forty-one received modified triangle hysterectomy;forty-one underwent hysterectomy only as control.The variables of operation and postoperation were studied and compared,especially those about the ovarian fuction and sexuality.Results:Significant differences were found in FSH,P,E2 level of the two groups.And there were significant differences in the time of flatus and urination,sexual fuction and menopausal symptom rate between the two groups.Conclusions:Modified trangle hysterectomy can be performed with less complications and slight influences on ovarian fuction,which could improve the quality of life of the patients.It has less impact on the urine and stool function.It resects the frequently-occurring part of cervical cancer;it can prevent from fallopian tube prolapse and vaginal nub prolapse,and avoid residual ovary syndrome.So it is valuable in clinical application.
