摘 要:目的:观察家兔机械通气所致肺损伤过程中MDA、TAC水平变化,探讨氧化应激对家兔机械通气所致肺损伤的影响。方法:16只家兔随机分为常规潮气量+PEEP组(C组)、机械通气所致肺损伤组(VILI组),每组8只;通气4 h放血处死动物,测定肺组织MDA及TAC含量,观察肺组织形态变化并进行肺损伤评分,测定肺组织湿干比值。结果:与C组比较,VILI组肺组织MDA值升高,TAC值降低,肺湿干比增加,肺组织出现明显损伤改变、肺损伤评分升高,以上差异均具有统计学意义。结论:致伤性通气方式能引起肺脏明显的氧化应激,而肺内的抗氧化能力相对不足,是机械通气所致肺损伤的重要机制。Objective:To explore changes of MDA and TAC levels in VILI and to study the oxidative stess in the pathogenesis of VILI.Methods:Sixteen new Zealand rabbits were randomly divided into 2 groups:(1)conventional ventilation + PEEP group(C group);(2)ventilator-induced lung injury group(VILI group).The levels of MDA and TAC in the lung tissue were assayed.Lung histopathology was observed and lung injury score was counted.Lung wet-to-dry weight ratio(W/D) was measured.Results:Compared with C group,MDA level,as well as W/D ratio and lung injury score in VILI group were increased,while TAC level was decreased.Conclusion:Harmful ventilation can cause oxidative stress in the lung,while antioxidative ability was insufficient,oxidative-antioxidative imbalance was the important mechanism of VILI.
