横行带蒂包皮瓣包绕及膀胱黏膜尿道成形术治疗尿道下裂

摘　　要：目的:探讨横行带蒂包皮瓣包绕及膀胱黏膜治疗尿道下裂的疗效。方法:本组78例尿道下裂患者:47例采用横行带包皮瓣尿道成形术;31例采用膀胱黏膜尿道成形。结果:上述两种方法手术时间平均为(4±0.7)h,横行带包皮瓣40例I期修复成功率为85.1%,4例出现尿瘘;膀胱黏膜尿道成形I期修复28例,成功率为90.3%,1例出现尿瘘,1例出现尿道憩室。1例尿瘘均于6个月后1次修补治愈,尿道憩室经憩室切除及尿道修补术后治愈。结论:横行带包皮内板或膀胱黏膜行尿道成形术是治疗尿道下裂的一种较好的方法;两种手术方式的结合运用可适应于各类需尿道成形术治疗的尿道下裂。Objective:To investigate the feasibility and efficacy in treatment of hypospadias by perfirming an operation on transverse pedicled flap wrapping packages and bladder mucosa.Methods:In 78 cases with hypospadias,Forty-seven cases were treated with free prepuce and 1 1 cases with bladder mucosa graft urethroplas.Results:The average operation time for the two ways was(4±0.7) h,the 40cases for urethra forming operation on transverse pedicle flap bag had 85.1%,of cures at stage 1 and 4 cases had urinary fistula;The 28 cases for urethra forming operation on bladder mucosa had 90.3%,of cures at stagel,1 case had urinary Diverticulum Each case with urinary fistula recovered 6 monthra Diverticulum excision and urethra repair.Conclusion:The bladder mucosa graft or internal preputial flap urethroplasty is the best procedure for different type of hypospadias repair in children.
