气管内置管注药治疗异物性支气管肉芽肿26例  

TREATMENT OF 26 BRONCHUS GRANULOMA CAUSED BY FOREIGN BODY WITH MEDICATION IN TRACHEA

摘　　要：目的 :评价气管内置管注药对异物性支气管肉芽肿的治疗效果。方法 :应用纤维支气管镜 (纤支镜 )气管内留置 6F导管连续每日注药治疗异物性支气管肉芽肿患者 2 6例 ,每日经导管分 2次注入无菌生理盐水10ml+2 %利多卡因 5ml+地塞米松 5mg +抗生素 (阿米卡星 0 .2或头孢三嗪 1.0 )。结果 :2 6例患者中 ,治愈2 5例 ,手术取异物 1例 ,治愈率 96 .2 % ,疗程 6～ 15d ,不良反应轻。结论 :该方法具有局部抗炎效果好 ,减少全身抗生素剂量和毒副作用 ,缩短病程 ,病人能够耐受等优点 ,有望临床推广应用。Objective:To evaluate the therapeutic effect of bronchus granuloma caused by foreign body with medication in trachea. Methods:In 26 patients suffered with bronchus granuloma caused by foreign body, 6F tube was put in sick trachea with fibrobronchoscope, and sterile Saline 10ml+2% lidocaine 5ml+dexamethasone 5mg+ antibiotic (Amikacin0.2 or ceftriazone1.0) were injected in sick trachea through 6F tube twice every day. Results:In 26 patients, 25 were cured, the cure rate was 96.2%,and the other one had to be operated; the course was 6～15d, the adverse reaction were slight.Conclusions: This method has virtues of good effect in local anti-inflammation, decreasing the antibiotic dosage and side effect of total body, shortening the course, and was tolerated by patients, our study suggests that this method should be applicated in near future.
