摘  要:探讨母亲妊娠合并糖尿病及妊高征对高危儿血糖、胰岛素、皮质醇、胰岛素样生长因子1水平(IGF-1)的影响,检测糖尿病组42例,妊高征组144例,健康对照组23例,均于出生24~48小时内采取股静脉血,测定有关内分泌水平。结果显示:(1)糖尿病组胰岛素、皮质醇水平较对照组显著增高,P<0.05;(2)妊高征组皮质醇水平明显高于对照组,P<0.05;(3)糖尿病与妊高征二组比较,IGF-1有显著性差异,P<0.05。结论,妊娠合并糖尿病及妊高征均对围产儿内分泌产生影响,可引起血皮质醇水平明显升高,尤其以前者为甚,可发生高胰岛素血症及血中IGF-1水平升高,胎儿柯兴氏面容、RDS及巨大儿的发生显著增高,影响围产儿的质量,应引起重视。To investigate the effect of sugar, insulin, cortisol , insulinoid growth factor 1(IGF-1) in neonates of diabetic mother and mother with pregnancy-induced hypertension syndrome (PIH) on perinate, blood sugar, insulin, cortisol, insulinoid growth factor 1(IGF-1) level were detected for prospective clinical study by taking femoral venous blood in 24- 48 hours after birth in 42 neonates of diabetic mother, 144 neonates of mothers with PIH and 23 normal neonates in controlled group. Results showed: (1) Insulin and cortisol level in 24-48 hours after birth in neonates of diabetic mothers were significantly higher thancontrolled group, P < 0. 05; (2) Cortisol level in PIH group was significantly higher than controlled group, P<0. 05; (3) There was significant difference in IGF-1 level between neonates of diabetic mothers and mothers with PIH, P < 0. 05. Conclusion: Diabetes in pregnancy and PIH syndrome have effect on endocrine of perinate, causing obvious increase of blood cortisol, especially the former may cause hyperinsulinism and of blood IGF-1 level which may cause significant increase of fetal Cushingoil-appearing, RDS and giant infant and effect the perinates. These should be payed attension.
