摘 要:目的 评价复合珊瑚羟基磷灰石(珊瑚羟基磷灰石/rhBMP-2/几丁糖,combined coralline hydroxyapatite,CCHA)在颈椎融合中的成骨效果。方法 按照序贯实验设计方法设计,以实验犬为实验对象,同体对照研究,分别选择颈椎3/4和5/6为融合节段,随机选择其中一个节段应用CCHA为植入物,另一个节段则选择自体髂骨为植入物,按照Robinson’s法进行椎间融合手术,并加钢板内固定。手术后10周进行X线检查,并按照Lane-Sandhu评分法中骨形成的评定标准来确定愈合情况,以骨融合情况为观察指标,进行统计、分析,并选定α=0.05,β=0.05,γ=FS/SF=2时,接受CCHA优于自体骨。结果CCHA应用于颈椎融合时,在手术后10周时,达到完全愈合,去除结果相同的1例,第12例时,曲线与上界相交,停止实验。结论 从手术后10周的骨融合情况来看,CCHA比自体骨融合效果更好。Objective To evaluate the effect of combined biosynthetic bone as substitute for iliac crest in the operation of cervical anterior interbody fusion in a canine model. Methods The trial was designed as sequential one. Each model received a combined biosynthetic bone and autogenous bone graft in the C3-4 or C5-6 disc interspaces at random and the treatments were performed by Robinson's procedune including internal fixation with plates. Plain radiographs were taken 10 weeks after operation and the pictures were grade by Lane - Sandhu' s methods. Thed biosynthetic bone was auepted when a=0.05, β=0.05, and γ=F( failure) S(suc-cess)/SF=2. Results The biosynthetic bone - combined coralline hydroxyapatite(CCHA) fused better in the cervical anterior fusions of experimental dogs at 10 weeks. Conclusion CCHA can be accepted as the substitute of iliac crest autogenous graft.
