摘　　要：目的　探讨 2型糖尿病合并自主神经病变患者与单纯性运动性高血压的关系。方法　随机选取血压正常的2型糖尿病患者 80例 ,按标准心血管自主神经功能试验 (SCANFT) (以下简称神功试验 )检查结果分为糖尿病自主神经病变 (DAN)组 (38例 )和单纯性糖尿病 (SDM)组 (42例 ) ,与自主神经功能检查阴性的 4 0例健康组作对比分析 ,并分析其静态血压、运动负荷后血压及动态血压变化。结果　三组静态血压无明显差异 ,DAN组运动负荷后的血压及运动性高血压发生率明显高于SDM组及对照组 (均P <0 0 1)。动态血压显示 :糖尿病患者 2 4hSBP、dSBP、nSBP均高于对照组 (P <0 0 1) ,而DAN组的 2 4hDBP、dDBP、nDBP及心率较SDM组及对照组显著增高 (P <0 0 1) ,结论　自主神经功能异常的糖尿病患者 ,运动负荷后的血压明显升高 ,运动性高血压可作为糖尿病患者合并自主性神经病变的预测指标之一。Objective To investigate the clinial significance of exercise induced BP response in patients with diabetic autonomic neuropathy. Methods We selected 80 patients with type 2 diabetes and 30 health controls for the study. According to the results test of cardiovascular autonomic neuropathic function(SCANFT), the patients with type 2 diabetes were divided into two groups: 38 case diabetic autonomic neuropathy (DAN) and 42 diabetes without complications(SDM). Treadmill test and 24 h ambulatory blood pressure were examined in the three groups. Results No significant difference in resting blood pressure was found in three groups. The exercise induced BP response were significantly higher in the DAN group. In 55.3% of DAN BP respionse was higher than the normal upper limit, while only 11.9% in SDM and 12.5% in control. Average 24 h, daytime and nighttime systolic blood pressure were significantly higher in diabetes ( P <0 01), but no difference were found between the SDM and DAN. Average 24 h, daytime and nighttime diastolic blood pressure were significantly higher in DAN ( P <0 01), than that in controls but no difference were found between the SDM and the control. Conclusion Patients with diabetic autonomic neuropathy are associated with hygher incidence of exercise induced hypertension. Exercise hypertension may be helpful to predict diabetic autonomic neuropathy.
