睑板前联合筋膜重建符合生理特征的重睑术

摘　　要：目的 通过对睑板前联合筋膜组织的解剖特点分析,探讨其在符合生理特征的重睑术中的临床意义。方法解剖显示上睑提肌向下延伸的联合筋膜将位于其外层的皮肤肌肉与深面的睑板粘连在一起,起着上睑提肌与眼轮匝肌到睑板前各层组织的动力传递作用。在切开重睑形成术中采用切口下唇皮缘直接与联合筋膜相缝合的方式形成重睑皱襞。结果通过对125例单睑患者施行重睑成形术,术后平均7～10天局部消肿,1～2个月恢复自然,重睑弧度流畅,在睁眼时显现生动活泼的重睑线,而闭眼时重睑线不明显,极似天然的双眼皮形态。随访3个月～年,均获满意效果。结论与传统重睑成形术相比,这种方法可提供一种符合生理特征的重睑成形术,值得推广与应用。Objective To investigate the anatomical charactistics of the conjunctional fascial tissue over tarsal plate and its clinical significance in the double eyelidplasty.Methods Anatomy research shows that the conjunctional fascial tissue originated by levator muscle has a firm attachment to the anterior surface of the tarsus and the overlying orbicularis oculi muscle and skin.The conjunctional fascial tissue plays a role in dynamic transmitting through the orbicularis muscle and the levator palpebrae superioris aponeurosis to all tissue over tarsal plate.We sutured eyelid skin below the incision directly with conjunctional fascial tissue over tarsal plate to form double eyelid fold.Results A total of 1256 cases received double eyelidplasty with satisfactory results.The follow-up period ranged from 3 months to 4 years.Edema diminished about 7～10 days after operation and the contour of the upper lid fold became natural within 1～2 months.Conclusion Compared with traditional double eyelid plasty,this technique makes double eyelid fold based on the mechanism of normal eyelid fold.
