异丙酚-普鲁卡因复合麻醉用于腹部手术麻醉的临床观察

摘　　要：目的　旨在充分发挥异丙酚起效快、作用时间短、苏醒迅速完全的基础上减少其用量 ,探讨异丙酚 普鲁卡因复合与单纯异丙酚用于腹部手术时麻醉强度、麻醉后恢复质量、及异丙酚与普鲁卡因配伍的可行性。方法　 4 0例成年腹部手术患者分成两组 ,A组以异丙酚 5mg·kg- 1 ·h- 1 、B组以异丙酚 2 .5mg·kg- 1 ·h- 1 +普鲁卡因 10mg·kg- 1 ·h- 1 静注维持麻醉 ,术中视麻醉深度辅以芬太尼、维库溴铵。结果　单位时间内两组芬太尼、维库溴铵用量 ,术毕呼吸功能恢复参数及初醒与拔管时间均无显著差别 ;而异丙酚用量A组是B组的一倍 ;B组病人初醒后有明显的嗜睡。结论　异丙酚 普鲁卡因复合液与芬太尼、维库溴铵配合使用 ,不仅可满足腹部手术的麻醉要求 ,且麻醉后恢复质量较高。B组异丙酚用量成倍减少使麻醉成本明显降低。Objective This paper aims at discussion on anesthesia strength and recovery quality after anesthesia in the combination application of propofol and procaine and procaine and the alone application of propofol as well as the feasibility of propofol compatible with procaine in abdomen operation on the basis of giving full play to the actions of propofol in the fast production of desired result, short acting time and rapid complete awaking. Methods Experimental methods include that 40 cases of patients subjected to adult abdomen operation are divided into two groups: group A with propofol 5mg·kg -1 ·h -1 +procain 10 mg·kg -1 ·h -1 to maintain anesthesia, supplemented with fontanel and vecuronium bromide according to the depth of anesthesia in the operation.Results Experimental results show that there are no significant difference in the dosages of fantayl and vecuronium bromide within unit time and the recovery parameters of respiratory function after the end of operation as well as the time for the beginning of awaking and extubation time between two groups; where as the dosage of propofol in group A is double that of propofol in group B; and patients in group B have the tendency of obvious lethargy after the beginning of awaking. Conclusion It is concluded that the application of the compound solution of propofol procaine in combination with fontanel and vecuronium bromide not only can satisfy the demand of anesthesia in abdomen operation, but also have rather high recovery quality after anesthesia. The decrease of propofol in dosage in group B can results in obvious decrease by 50% in the cost of anesthesia.
