儿童消化性溃疡与幽门螺杆菌感染

摘　　要：①目的　探讨儿童消化性溃疡与幽门螺杆菌 (Hp)感染的关系及临床特征。②方法　经OlympusGIF XP2 0型纤维胃镜检查 ,取病变部位黏膜活检。Hp感染诊断标准 :于胃窦大弯或后壁距幽门 3～ 5cm内取两块黏膜进行组织切片 ,姬姆萨染色镜下检菌 ,行Hp UT药盒快速尿素酶试验及血清抗Hp抗体检测 ,其中 2项阳性则确诊为Hp感染。③结果　球部溃疡 1 2 0例 ,胃溃疡 2 1例 ,复合溃疡 6例。全部病儿均并发程度不等的慢性浅表性胃炎。Hp检测显示 1 4 7例病儿中尿素酶试验阳性 1 1 1例 (75 .51 % ) ;1 2 9例行姬姆萨染色镜下检菌 ,1 0 8例阳性 (83 .72 % ) ;36例行血清抗Hp抗体检测 ,2 1例阳性 (58.33 % )。 4～ 1 0岁组 ,Hp阳性率 (61 .1 1 % )明显低于 >1 0岁组 (83 .87% ) ,差异有极显著意义 (χ2 =9.57,P <0 .0 1 )。④结论　Hp感染与小儿消化性溃疡密切相关。Hp感染随年龄增长而增高。Objective\ To study the relationship between peptic ulcer and helicobacter pylori(Hp) infection and its clinical features in children.\ Methods\ The patients were examined by fiber gastroscopy, and a biopsy was done simultaneously for lesioned mucous membrane. Diagnostic criteria of Hp infection, If two in three tests giemsa stain of diseased tissue, rapid urease test and determination of serum anti Hp antibody were positive, the diagnosis of Hp infection was established. \ Results\ The diagnosis of duodenal ulcer(120 cases), gastric ulcer(21 cases ) and complex ulcer( 6 cases) was confirmed. All cases were complicated with chronic superficial gastritis to certain extent. The urease test showed positive in 111 cases(75.51%);Giemsa stain for Hp , 108 cases being positive (83.72%); the positive rate of serum anti Hp antibody was 58.33%(21 cases). Hp positive rate was markedly lower in 4 to 10 year old group(61.11%) than >10 year old group(83.87%)( χ 2=9.57, P <0.01).\ Conclusion\ Hp infection is closely associated with the peptic ulcer in children and with the growing of the age.
