不同剂量冻干精制Vero细胞流行性乙型脑炎灭活疫苗人体反应及血清学效果观察

摘　　要：为了解不同剂量冻干精制Vero细胞流行性乙型脑炎 (乙脑 )灭活疫苗 (Vero细胞乙脑疫苗 )接种后的人体反应和血清学效果 ,2 0 0 0年 3～ 4月在苏州市吴中区 ,将 2 4 3名 6～ 12月龄儿童分为A、B、C 3组 ,接种不同剂量的Vero细胞乙脑疫苗 ;以 114名同龄健康儿童接种地鼠肾乙脑灭活疫苗为对照。观察接种后 6h、2 4h、4 8h的发热反应和局部反应 ,并在免疫前和免疫后 1个月采血 ,检测中和抗体 ,以≥ 1∶5为阳性。首针接种后A组 (2针均为0 5ml疫苗 )中、强发热反应达 13 3% ,明显高于B组 (首针 0 2 5ml,第 2针 0 5ml)、C组 (2针均为 0 2 5ml)和对照组的 2 4 %、3 1%和 2 6 % ,B组、C组与对照组无明显差别 ;血清学检测A组和B组中和抗体阳转率达 10 0 0 %和95 2 % ,抗体几何平均滴度 (GMT)为 1∶2 7 7和 1∶12 9,明显高于C组和对照组的阳转率 81 7%、6 8 6 %和GMT1∶8 5 2、1∶7 0 7;A组与B组抗体阳转率无明显差异。说明Vero细胞乙脑疫苗全身反应和局部反应小 ,接种 2针后免疫学效果良好 ,但首针全量注射后中、强发热反应者较多 ,采用首针半量接种能大大减少发热反应 。In order to learn of the adverse reaction and immunological efficacy of primary immunization with different doses of inactivated freeze-derived Vero cell Japanese encephalitis(JE)vaccine,in March～April 2000,from Wuzhong District of Suzhou City,243 healthy children of 6-12months old who had no history of JE vaccination and JE illness were selected and divided into 3 groups(A,B,C)receiving different doses of Vero cell JE vaccine.114 healthy children of the same age were set up as control(group D) receiving hamster kidney cell JE vaccine.The local reaction and fever were observed at 6h,24h,and 48h after vaccination,the blood simples were collected before and after one month of injection for detection of neutralizing antibody taking titer ≥1∶5 as positive.The results showed that 13.3% of the children in group A(receiving 2 doses of JE vaccine,0.5ml each)had moderate and severe fever as compared with 2.4% of group B(receiving 2 doses of JE vaccine,0.25ml,0.5ml)3.1% of group C(receiving 2 doses of JE vaccine,0.25ml,0.25ml)and 2.6% of group D. The positive seroconversion rates and GMTs of groups A and B were 100%,95.2% and 1∶27.1,1∶12.9 respectively,which were statistically higher than 81.7%,68.6% and 1∶8.52,1∶7.07 of groups C and D respectively.The systemic and local reactions of Vero cell JE vaccine were mild,and the immunological efficacy was good.Because of the incidence of moderate and severe fever seen in group A,it is recommended to immunize with a first dose of 0.25ml and then a second dose of 0.5ml of JE vaccine which can reduce the fever reaction and get better immune effect.
