经静脉声学造影对急性肾衰肾皮质血流灌注与血肌酐、尿素氮变化关系的实验研究

摘　　要：目的 :探讨兔急性肾功能衰竭 (ARF)时肾皮质血流灌注与血肌酐、尿素氮变化的关系。方法 :兔后腿肌注 50 %甘油 (1 2～ 1 5ml/ kg) ,建立 ARF动物模型 ,分别于注射甘油前 (T0 )、注射甘油后 1、 2、 3、 5、 7… 1 3天 (T1 、T2 、T3 、T5… T1 3 )共 9个时间点采用经静脉声学造影和声学密度 (AD)定量分析技术 ,测定肾皮质造影后峰值密度 (PI)、曲线下面积 (AUC)等有关灌注参数 ;并同时抽血查肌酐 (SCr)、尿素氮 (BUN) ,将化验结果与皮质灌注参数作相关性分析。结果 :肾皮质造影后 PI、AUC和 SCr、BUN均于 T1 即开始变化 ,PI、AUC于 T3 降低到最低值后即开始升高 ,至 T9时已基本恢复正常 ;而 SCr、BUN在 T5达到其最高值后仍在较高水平上保持至 T9,下降缓慢 ,至 T1 3 才接近正常水平。从 T0 ～ T1 3 ,PI、AUC与 SCr的相关系数分别为 0 .66、 0 .58,与 BUN的相关系数分别为 0 .64、 0 .61。结论 :急性肾衰时 ,PI、 AUC与 SCr、 BUN均变化较早 ,但 PI、 ACU异常持续时间短 ,恢复较快 ;而 SCr、 BNU异常持续时间较长 ,恢复较慢。从 T0 ～ T1 3 ,PI、 AUC与 SCr、Objective:To explore the relationship between the serum creatinine(SCr),blood urea nitrogen(BUN)levels and the blood perfusion of renal cortex in acute renal failure(ARF).Methods:The model of ARF was established by the intramuscular injection of 50% glycerol(12 15ml/kg)into the hind limbs of 18 rabbits.The levels of SCr and BUN were measured on the day before injection(T 0)and the days 1,2,3,5...11,13 after injection(T 1,T 2,T 3,T 5...T 11 ,T 13 while ultrasonic examination was performed by intravenous contrast ultrasound and acoustic densitometry(AD)technique to acquire some perfusion parameters of renal cortex such as peak intensity(PI)and the area under the curve(AUC).Then,the correlation of examination results was analyzed by statistic mehtods.Results:The PI and ACU of renal cortex after injecting contrast agent and SCr,BUN levels have changed greatly at T 1 compared to T 0.The PI,AUC were decreased to their lowest values at T 3 and then began to be increased and reached to thier normal levels at T 9 finally.The SCr and BUN were increased to thier highest levels at T 5 and remained at this high level to T 9 and then began to be decreased slowly.From T 0 to T 13 ,the relative coefficients of PI and AUC of renal cortex after injecting contrast agent to SCr were 0 66 and 0 58 respectively.The relative coefficients of PI and AUC of renal cortex after injecting contrast agent to BUN were 0 64 and 0 61 respectively.Conclusions:During ARF stage,PI,AUC,SCr,and BUN all changed earlier,but the PI,AUC maintained abnormal shortly and recovered rapidly.The SCr and BUN remained at abnormal levels for a longer time and recovered slowly.From t 0 to T 13 ,the PI and AUC values of renal cortex did not correlate to SCr and BUN levels.
