E—选择素、CEA联合检测对良恶性胸腔积液的鉴别

摘　　要：目的 探讨血清和胸腔积液中E-选择素（E-selectin或E-SLT），癌胚抗原（CEA）及胸腔积液细胞学多项指标联合检测对结核性和恶性胸腔积液的诊断价值。方法 对35例结核性胸腔积液，35例恶性胸腔积液及20例健康人的血清和胸腔积液进行检测。结果 恶性胸腔积液组血清和胸液中E-SLT低于结核性胸腔积液组，而CEA则前者高于后者，差异均有高度显著性。以胸液E-SLT>23ng/ml为结核组的界值，单项指标诊断结核性胸腔积液的敏感性为91.4%，特异性71.4%，准确性81.4%，以血清CEA5μg/ml为恶性胸腔积液界值，敏感性为68.6%。特异性91.4%，准确性80%，联合上两项指标加胸腔积液细胞学检查，敏感性97.1%，特异性100%，准确性98.6%，结论 E-SLT及CEA联合检测对良恶性胸腔积液的鉴别有重要意义。Objective\ To evaluate the significance of E-selectin and CEA in serum and pleural effusions in differentiating tuberculous and malignant pleural effusion. Methods\ 35 cases of malignant pleural effusions and 35 cases of tuberculous pleural effusion and 20 cases of healthy people were included.Results\ E-selectin levels of serum and pleural effusions from malignant pleural effusions group were significantly lower than those from tuberculous group. CEA levels of serum and pleural effusions in malignant group were significantly higher than those from tuberculous group ( P<0.01). E-selectin level of pleural effusion above 23 ng/ml was regarded as the cutoff value for the confirmed diagnosis of tuberculosis, with the sensitivity 91.4%, the specificity 71.4%, the accuracy rates 81.4%. CEA level of serum above 5 μg/ml as the cutoff value for the confirmed diagnosis of malignant pleural effusions, with the sensitivity rates 68.6% ,the specificity 91.4%,and the accuracy rates 80%. Combination of the two index.Conclusion \ E-selectin and CEA simultaneous detection was helpful for differentiating benign and malignant pleural effusion.
